后囊环形撕囊制作起始瓣的新体会

摘　　要：目的　探讨后囊环形撕囊制作起始瓣对玻璃体前界膜完整性的影响。方法　对 4 2眼后囊膜增殖白内障手术于摘除白内障后在后囊膜旁中心处用破囊针头作表浅划痕 ,擦出起始瓣 ,再沿起始瓣边缘撕开一直径约 3～ 4mm的圆孔 ,再植入人工晶体。结果　全部术眼玻璃体前界膜均完整 ,后囊膜中央均有一透明类圆孔。结论　经改良的后囊环形撕囊起始瓣的制作能安全有效地保持玻璃体前界膜的完整性 ,防止后囊环形撕囊术中玻璃体溢出。Objective To evaluate the effect of posterior continuous curvilinear capsulorhexis(PCCC) with initial tearing method on the integrity of anterior hyaloid membrane.Method PCCC were performed by initial tearing method after cataract extraction on 42 eyes with posterior capsular proliferative cataract.The tear was begun by making a puncture in the paracentral posterior capsule with a discission needle.The edge of the tear was guided by the needle to achieve a 3～4mm circular capsulectomy.IOLs were implanted after finishing PCCC.Result Anterior hyaloid membrane remained integrate and a circular capsulectomy on posterior capsule were abserved in all eyes.Conclusion Modified PCCC is safe and effective in protecting the integrity of anterior hyaloid membrane and preventing vitreous prolapse.
