宫颈锥切术后妊娠结局的分析及影响

摘　　要：目的:观察宫颈锥切术对育龄妇女妊娠结局的影响。方法:收治宫颈上皮内瘤变或原位癌实施宫颈锥切术后成功妊娠的患者32例为观察组。随机选择同期妇产科年龄<40岁无宫颈锥切术史产妇30例作对照组。结果:观察组实施宫颈锥切术与妊娠的间隔时间<1年的产妇9例,>1年24例。观察组自然分娩率显著低于对照组(25.00%VS53.33%,P<0.05)。观察组和对照组平均分娩孕周分别是37.6周和39.1周,差异无统计学意义(P>0.05)。平均产程两组相比差异无统计学意义[(492±114)min VS(523±109)min,P>0.05]。观察组早期流产2例、早产5例,对照组无早期流产发生,仅有1例早产,两组相比差异有统计学意义(P<0.05)。两组胎儿窘迫、胎膜早破、低体重等差异均无统计学意义(P>0.05)。结论:宫颈锥切术后育龄妇女妊娠后自然顺产率低于未手术者,但其妊娠结局无明显影响,值得临床应用。Objective:To observe the influence of cervical conization pregnancy outcomes of women of childbearing age.Methods:32 patients with cervical intraepithelial neoplasia or in situ carcinoma in pregnancy after cervical conization were selected as the observation group.At the same time,30 patients less than 40 years of age without cervical conization history puerpera were selected as the control group in obstetrics and gynecology department.Results:In the observation group,the interval time berween cervical conization and pregnancy of 9 cases was less than 1 year,and 24 cases were more than 1 year.In the observation group,the natural childbirth rate was significantly lower than that of the control group(25% VS 53.33%,P<0.05).The average gestational age of the observation group and the control group was respectively 37.6 and 39.1 weeks,and there was no significant difference(P>0.05).The average birth process of the two groups were compared,and there was no significant difference[(492±114)min VS(523±109)min,P>0.05].In the observation group,2 cases had early abortion,and 5 cases had premature labor;in the control group,there was no early abortion,and only 1 case had premature delivery;the difference was statistically significant(P<0.05).Fetal distress,preterm premature rupture of membranes,low body weight of the two groups were not statistically different(P>0.05).Conclusion:The natural birth rate of pregnancy women after cervical conization was lower than that without surgery,but other pregnancy outcome was no obvious difference.It was worthy of clinical application.
