摘　　要：目的　探讨早、中期口腔癌应用外照射合并192 Ir -HDR后装放疗的疗效。方法　对 16例早、中期口腔癌患者 ,首先给60 Co或 6MV -X加速器外照射 ,外照总剂量为Dt 4 6～ 5 6Gy ,2 3～ 2 8次 ,4～ 6周。外照结束后 1周 ,给192 Ir -HDR后装组织插植或肿瘤表面贴敷放疗 ,参考点剂量为Dt18～ 32Gy ,2～ 5次 ,3～ 15天 ,间隔 1～ 5天。 结果　 5年生存率为 81.3% ,无严重的放射后遗症。结论　对早、中期口腔癌 ,就用外照射合并192 Ir-HDR后装放疗疗效与手术疗效相似 ,无严重的放射后遗症 ,且患者能保持外观和正常组织功能 ,提高了病人生存质量。Objective To assess the effects of external beam radiotherapy combining with 192Ir-HDR brachytherapy in treating cancer of the oral cavity. Methods 16 patients with stageⅠ-Ⅲ carcinoma of oral cavity were treated with 60co r-ray or 6 MV X-ray. A total dose of 46-56 Gy was given at 23-28 fractions in 4-6 weeks , and 7 days later, 192Ir-HDR interstitial or mould brachytherapy was delivered to a reference dose of 18-32 Gy at 6-8 Gy per fraction, once every 3-5 days . Result The 5-year survival rate was 81.3%, and no severe radiation complications occured. Condusion 192Ir-HDR brachytherapy combined with external radiation is effective and has no severe complications in the treatment of carcinoma of oralcavity. This treatment can maintain normal appearance and function of the normal tissues.
