胃肠道B细胞性淋巴瘤形态及免疫组织化学分析

摘　　要：目的分析胃肠道B细胞性淋巴瘤的病理形态以及免疫组化特点。方法选取2016年5月至2018年5月我院确诊为胃肠道B细胞性淋巴瘤患者40例,制作病理标本进行组织形态学以及免疫组织化学染色分析。结果 40例患者中,男性25例,女性15例,发病年龄29~78岁,平均年龄(53. 3±2. 31)岁; 40例患者肿瘤细胞弥漫浸润胃肠壁全层、细胞构成以中心或免疫母细胞为主、其中19例出现"星空现象"。确诊为弥漫大B细胞淋巴瘤21例(52. 50%);套细胞淋巴瘤9例(22. 50%),黏膜相关组织淋巴瘤(MALT) 6例(15. 0%),不能分类者4例(8. 90%);免疫组化结果:40例患者的免疫组织化学染色显示所有病例均表达CD20以及CD79a,所有病例均无CD3表达,同时在不同类型的淋巴瘤均有不同程度表达bcl-2、Cyclin D1等。结论胃肠道是B细胞淋巴瘤结外最常发生的部位,主要分为大B细胞淋巴瘤以及小B细胞淋巴瘤,其明确诊断需要依靠临床资料以及免疫组化结果。Objective To analyze the pathological and immunohistochemical features of gastrointestinal B-cell lymphoma. Methods A total of 40 patients with gastrointestinal B-cell lymphoma diagnosed in our hospital from May 2016 to May 2018 were enrolled,pathological specimens were prepared for histomorphology and immunohistochemical staining. Results Of the 40 patients,25 were male and 15 were female. The age of onset was 29-78 years old,with an average age of( 53. 3±2. 31) years. In 40 patients,the tumor cells were diffusely infiltrated into the whole gastrointestinal wall,and the cells were mainly composed of central or immunoblasts. Among them,19 cases showed'starry sky phenomenon'. 21 cases( 52. 50%) were diagnosed as diffuse large B-cell lymphoma,9 cases( 22. 50%) with mantle cell lymphoma,6 cases( 15. 0%) with mucosa-associated lymphoma( MALT),and 4 patients( 8. 90%)were unable to be classified. Immunohistochemical results: Immunohistochemical staining of 40 patients showed that CD20 and CD79 a were expressed in all cases. CD3 expression was not observed in all cases,and bcl-2 and CyclinD1 were expressed in different types of lymphoma. Conclusion Gastrointestinal tract is the most common site of extracellular B cell lymphoma. It is mainly divided into large B-cell lymphoma and small B-cell lymphoma. The diagnosis depends on clinical data and immunohistochemistry results.
