摘  要:目的探讨3D-伪连续性自旋标记技术(3D-pASL)对于儿童颅内肿瘤的诊断价值。方法 25例儿童颅内肿瘤患者行3T常规磁共振检查和3D-伪连续性自旋标记检查。通过选择感兴趣区,获得肿瘤的最大相对血流量,并与其组织学特征和分级进行对照。结果 25例患者包括:10例星形细胞瘤,4例胚胎性肿瘤,3例室管膜瘤,3例脉络丛乳头状瘤,2例颅咽管瘤,2例淋巴瘤,1例胶质母细胞瘤。高级别肿瘤(Ⅲ、Ⅳ级)的最大相对血流量明显高于低级别肿瘤(Ⅰ、Ⅱ级)(P<0.01),除了淋巴瘤。本组病例显示相对血流量不能区分肿瘤的组织学类型,但是在后颅窝肿瘤,髓母细胞瘤的相对血流量要高于毛细胞星形细胞瘤(P<0.017)。结论 3D-伪连续性自旋标记对于评估儿童颅内肿瘤以及区分肿瘤级别是一种有价值的检查方法。Objective To explore the diagnostic value of 3-Dimensional pseudocontinuous arterial spin labeling in pediatric brain tumors.Methods Twenty-five pediatric patients(16boys and 9girls)with Brain Tumors,who were been histologically proven,were examined at a clinic with 3.0T MR imaging.The 3Dpseudocontinuous spin-echo arterial spin-labeling technique was acquired at 3T MR imaging.Maximal relative tumor blood flow was obtained by use of the ROI method and was compared with tumor histologic features and grade.Results Tumors consisted of astrocytic tumor(10),embryonal tumor(4),ependymal tumor(3),choroid plexus tumor(3),craniopharyngioma(2),lymphoma(2),glioblastoma(1).The maximal relative tumor blood flow of high-grade tumors(grades III and IV)was significantly higher than that of low-grade tumors(grades I and II)(P <0.01),except lymphoma.In this patients,relative tumor blood flow did not distinguish individual histology;however,among posterior fossa tumors,relative tumor blood flow was significantly higher for medulloblastoma compared with pilocytic astrocytoma(P <0.017).Conclusion 3-Dimensional pseudocontinuous arterial spin is a suitable method for assessment of pediatric brain tumors and allows distinction between high-and low-grade tumors.
