摘 要:目的探讨磁敏感成像对脑微出血的诊断价值,以及脑微出血的临床意义。方法 131例临床疑有脑卒中患者,通过常规MRI T1WI、T2WI、FLAIR及磁敏感成像检查,分析脑微出血灶分布及同时存在的脑内缺血、出血改变。结果发现脑微出血灶44例,共197个,好发部位依次为皮层—皮层下115个(58.38%)、基底节49个(24.87%)、脑干17个(8.63%)、丘脑8个(4.06%)、小脑8个(4.06%)。44例患者中32例脑内均有不同程度缺血性改变,表现为两侧基底节区、侧脑室旁白质区、脑干异常信号,T1WI呈等、低信号,T2WI、FLAIR呈高信号。其中15例患者合并急性脑梗死,4例患者出血性卒中合并脑微出血。结论磁敏感成像在检出脑微出血灶方面有明显的优势,脑微出血可能提示存在脑出血的危险。Objective To investigate the diagnostic value of MR susceptibility weighted imaging in the cerebral microbleeds(CMBs),and the clinical significance of the cerebral microbleeds.Methods Conventional MR T1 WI,T2WI,FLAIR and susceptibility weighted imaging were performed in totally 131 patients with suspected stroke,the distribution of the cerebral microbleeds was analyzed.Results CMBs were found in 44 patients,the number of CMBs were 197,and their location were the cortico-subcortical region in 115(58.38%)cases,basal ganglia in 49(24.87%)cases,the brain stem in17(8.63%)cases,thalamus in 8(4.06%)cases,cerebellum in 8(4.06%)cases.Among 44 patients,32patients were found lacunes on basal ganglia,white matter around sided ventricle,and the brain stem.There were acute infarction in 15 cases,hemorrhagic stroke with CMBs in 4cases.Conclusion Marked advantages were shown in the detection of CMBs by susceptibility weighted imaging.The presence of microbleeds may be a direct maker of advancing small vessel diseases,and may have a risk of bleeding.
