摘　　要：目的探讨PET-CT在肝泡型棘球蚴病与肝实性占位性病变鉴别诊断中的应用价值,旨在提高影像学对肝脏泡型棘球蚴病的诊断价值。方法收集肝实性占位性病变的患者资料57例,所有患者均做了PET-CT和普通CT,分别观察病灶在PET-CT和CT上的表现,根据影像学特征进行定性诊断和鉴别诊断。结果本组57例患者经手术或经皮穿刺活检证实23例肝脏泡型棘球蚴病,18例为胆管细胞型肝癌,7例转移瘤,5例肝脓肿,4例肝脏结核;PET-CT与普通CT对肝泡型棘球蚴病的检出率差异有统计学意义(P<0.05)。结论肝泡型棘球蚴病具有特征性的PET-CT表现,可作为与其他肝脏实性占位性病变的鉴别诊断依据。Objective To evaluate the value of PET-CT in the differential diagnosis of hepatic alveolar echinococcosis and hepatic solid space-occupying lesions, so as to improve the diagnostic value of imaging in hepatic alveolar echinococcosis. MethodsThe data of 57 patients with solid hepatic space occupying lesions were collected. All patients underwent PET-CT and conventional CT. The manifestations of the lesions on PET-CT and CT were observed respectively. The qualitative diagnosis and differential diagnosis were made according to the imaging features. Results Twenty-three cases of hepatic alveolar echinococcosis were confirmed by operation or percutaneous biopsy in 57 patients, including 18 cases of cholangiocarcinoma, 7 cases of metastatic tumor, 5 cases of liver abscess and 4 cases of hepatic tuberculosis. The detection rate of hepatic alveolar echinococcosis was significantly different between PET-CT and conventional CT(P<0.05). Conclusion Hepatic alveolar echinococcosis has characteristic PET-CT findings, which can be used as a basis for differential diagnosis with other solid space-occupying lesions of the liver.
