摘　　要：目的　探讨非小细胞肺癌 (non smallcelllungcancer,NSCLC)动态增强MRI指标在NSCLC的临床分期及淋巴结转移估测方面的价值。方法　分析 3 3例NSCLC患者的动态增强MRI资料 ,测量病灶的最大强化斜率 (Smax)及峰值到达时间(TTP) ,并对手术后的临床分期及淋巴结转移情况进行统计学分析。结果　 3 3例NSCLC ,Smax及TTP值分别为 0 .7～ 3 .0 5(1 40± 0 5)和 73～ 2 83s (14 3± 3 5.5s) ;早期患者的Smax及TTP分别为 1.2 1± 0 .3 0和 187.5± 2 8.96s ,中晚期患者则为 1.97±0 70和 96 0 0± 16 12s ,tSmax=3 .88,P <0 .0 1;z=-2 .63 ,P <0 .0 5;有淋巴结转移组的Smax及TTP平均值分别为 2 .14± 0 .69和 90 .5± 2 2 .55s ,无淋巴结转移组则为 1.2 6± 0 .3 3和 198.0 8± 2 6.88s,tSmax=3 .57,P <0 .0 1;z =-3 .46,P <0 .0 0 1。结论　动态增强MRI有关指标有助于肺癌的诊断及分期 。Objective To evaluate clinical staging and lymph-no de metastases by dynamic contrast-enhanced MR imaging in non-small cell lung c ancer(NSCLC). Methods Thirty-three consecutive patients firstl y found mass by plain film or CT wereperformed with non-contrast conventional M R imaging, dynamic contrast-enhanced scan. Some parameters of DCE MRI, includin g maximum slope(Smax) and time to peak(TTP) were compared with their clinical st aging and lymph-node metastases. Results The Smax was 0.7～3. 05(1.40± 0.5) ,and TTP was 73～283s (143±35.5s).Smax in stageⅠwas lower than that in stage Ⅱand Ⅲ(1.21±0.30 and 1.97±0.70;t Smax =3.88, P <0.01), but TTP was later(187.5±28.96s and 96.00±16.12s;z= -2. 63, P <0.05). Smax in NSCLC with lymph-node metastases was higher than that without lymph-node metastases(2.14±0.69 and 1.26±0.33,t Smax =3.57, P <0 01), but TTP was earlier(90.5±22.55s and 198.08±26.88s,z=-3.46, P <0 .001). Conclusion Parameters o f DCE MRI, especially Smax and TTP, correlate to lymph-node metastases closely, can help lung cancer diagnosing, staging, and serve as a supplement to the curr ent staging system for lung cancer in biological behaviour.
