无结膜切口的小梁切除术术式改良及疗效观察

摘　　要：目的对无结膜切口的小梁切除术进行改良,探讨其可行性、有效性和安全性。方法采用倾斜巩膜隧道、楔状小梁切除、加大滤过内外口等措施对原术式进行改良,临床应用于30例32眼青光眼。结果术后结膜反应轻微,前房开始时稍浅,术后7d恢复正常,术后随访12～37个月眼压均在正常范围,滤过泡弥散、扁平、范围广,均为功能性滤过泡。结论该术式操作安全、效果良好、并发症少。Purpose We modified the trabeculectomy without conjunctival incision and investigated its feasibility, effectiveness and safety. Methods The technique was modified with slope scleral tunnel, wedge trabeculectomy and enlarged filtering openings. 32 eyes of 30 patients received the operation. Results In a few days after the operation the conjunctiva was found with slight reaction and shallow anterior chamber was observed in all case but returned to mormal within 1 week. IOP was normal during the following 12 ~ 37 months period without any medication.The filtering bled-was diffuse,flat and well functioned. Conclusion The modified technique is safe,effective and with fewer complications.
