摘 要:目的研究基质金属蛋白酶(matrixmetalloproteinase,MMP)及其抑制剂(tissueinhibitorofmetalloproteinase,TIMP)在成釉细胞瘤(ameloblastoma,AB)中的表达,结合临床病理,探讨其与生物学行为的关系。方法用免疫组化SP法检测43例AB的MMP-2、MMP-9、TIMP-1的表达,同时以口腔黏膜8例、牙源性角化囊肿(odontogenic,keratocyst,OKC)10例为对照。结果MMP-2、MMP-9在正常黏膜为阴性或弱阳性表达,在OKC棘层细胞中强阳性表达2例,阴性或弱阳性8例,基底层细胞表达较弱或阴性。在AB的外周层及星网状层细胞中,强阳性表达28例和30例(在MMP-2中组间相比P<0.001)。MMP-9中AB与正常黏膜及OKC比较,P<0.05。MMP在AB间质细胞表达为阳性,且随组织学分级变化而增高,但与年龄、性别、部位、病理分型无关(P>0.05)。TIMP-1在正常黏膜、间质组织、AB及OKC中均为弱阳性或阴性表达。结论MMP-2和MMP-9蛋白的高表达与AB的复发有关,MMPs与TIMPs的蛋白表达失平衡,可能为AB侵袭性生长的重要因素之一;基质细胞产生的MMPs蛋白活性,也可能为AB利用且在侵袭性生长中起到重要作用。Objective To study the expression of matrix metalloproteinase(MMP)and tissue inhibitor of metalloproteinase(TIMP)in ameloblastoma(AB)and to determine the relationship between biological behavior of AB and clinical pathology.Methods The specimens of43cases of AB,10cases of odontogenic keratocyst(OKC),8cases of normal oral mucosa were examined by streptavidin-biotin method.Results In8cases of normal oral epithelial,MMP-2was negative or weak positive.In OKC,MMP-2was extensively positive in stratum spinosum of2cases,and weekly or negative in stratum basale.MMP-2was strongly expressed in the central and peripheral cells of the tumor islands in28cases of AB.There were difference among these three groups(P<0.001).Comparing AB with normal oral mucosa and OKC,there was signifi-cant difference(P<0.05).MMP was positive expressed in cells of stroma.The positively rate and intensity increased as AB recurrene and transformed malignantly,but were not associated with age,sex,pathological type and location.TIMP-1was weakly or not expressed in normal oral mucosa,stoma,OKC and AB.Conclusions The high expression of MMP-2and MMP-9is related to the biological behavior of AB.Imbalances of the expression of MMP-2and MMP-9protein maybe be one of the facts of the invasion of AB.The MMPs activation produced by stromal cells may be also be related to the inva-sion of AB.
