边疆民族地区基层基本医疗服务改革实证研究

摘　　要：目的:调查边疆民族地区基层基本医疗卫生服务改革进展,探讨加强服务保障能力与水平的路径。方法:随机抽取乌鲁木齐市4个区19所基层医疗卫生机构进行调查研究。结果:乌鲁木齐市基层医疗卫生机构有4种举办主体,其中非政府举办占89.8%,政府举办占10.2%;基层医疗卫生机构医护比仅为1∶0.96,远低于1∶2的国际水平;社区卫生服务机构近三年年均门诊量无明显变化;新农合参合率达到99.9%。基药制度实施进展:社区卫生服务中心达到88.9%,社区卫生服务站达到62.5%。结论:乌鲁木齐市基层基本医疗卫生服务公共财政投入明显增加,服务规模明显扩大,基层群众就医方便程度明显提高,但服务能力建设还亟待加强。Objective: To investigate the progress of the basic medical service reform in base frontier minority areas, and to explore the path to strengthen the capability of the base medical. Methods: A simple sample was used to randomly select 19 of basic medical and health institutions of four districts in Urumqi.Results: The basic medical and health institutions in Urumqi were held by four different main bodies, the nongovernmental bodies account for 89.8%, and only 10.2% of basic medical and health institutions were held by the government. The medical ratio of basic community health service institutions was only 1∶0.96, and it was far lower than international standard which was 1 ∶2. The annual average outpatient amount of community health service institutions had no obvious change in nearly three years. The participation rate of new village cooperation medical treatment expenses reached 99.9% in rural. The progress of implementation of basic drug homodyne rate system in urban and rural basic medical and health institutions was listed as follows: community health service center reached 88.9% and community health service station reached 62.5%. Conclusion: The public finance of the basic medical and health services increase obviously, the scale of expanded significantly. It is more convenient for the masses of basic level to take medical treatment. However, the capacity of basic medical and health services institutions should be sharply strengthened.
