经颅钻孔引流联合Ommaya管置入术治疗囊性胶质瘤临床疗效分析

摘　　要：目的探讨经颅钻孔引流联合Ommaya管置入术治疗囊性胶质瘤的临床疗效。方法将符合诊断标准的62例囊性胶质瘤患者,采用数字表法随机分成观察组(31例)和对照组(31例),观察组患者给予经颅钻孔引流联合Ommaya管置入术进行治疗,对照组患者给予三维适形放疗联合三苯氧胺进行治疗。比较2组患者临床症状的改善情况、影像学复查情况以及不良反应发生情况。结果观察组患者临床症状消失率为41.9%(13/31),显著性高于对照组的19.4%(6/31),差异有统计学意义(X^2=3.71,P<0.05)。2组患者总有效率分别为77.4%(24/31)与61.3%(19/31),不存在显著性差异(P>0.05)。观察组患者的影像学总有效率为89.7%(26/29),显著性高于对照组的67.9%(19/28)(X^2=4.07,P<0.05),观察组显效率与有效率分别为41.4%与48.3%,与对照组的28.6%与39.3%相比,差异无统计学意义(P>0.05)。观察组患者颅内压增高率与不良反应总发生率分别为3.4%与20.7%,均显著性低于对照组的25.0%与46.4%(X^2=5.48,4.25,P<0.05)。结论经颅钻孔引流联合Ommaya管置入术治疗囊性胶质瘤安全有效、疗效显著。Objective To investigate the clinical efficacy of transcranial drilling drainage combined with Ommaya tube implantation in the treatment of cystic glioma.Methods Sixty-two cases of cystic glioma patients were randomly divided into observation group(31 cases) and control group(31 cases) using digital table,observation group were treated with transcranial drilling drainage combined with Ommaya tube placement,the control group were treated with three dimensional conformal radiotherapy combined with tamoxifen.Compared the improvement of clinical symptoms,the image examination and adverse effect between the 2 groups.Results The clinical symptoms in observation group patients disappearance rate was41.9%(13/31),significantly higher than that in the control group of 19.4%(6/31),the difference was statistically significant(x^2 =3.71,P <0.05).Two groups of patients’ total effective rate were 77.4%(24/31) and 61.3%(19/31),no significant differences were found(P > 0.05).The imaging total effective rate in the observation group was 89.7%(26/29),which was significantly higher than that in the control group of 67.9%(19/28)(x^2 =4.07,P <0.05).The success rate and effective rate in the observation group were 41.4%and 48.3%sepreately,compared with 28.6%and 39.3%in the control group sepreately,there was no significant difference(P >0.05).Intracranial pressure and adverse reaction rate in the observation group were 3.4%and 20.7%respectively,which were significantly lower than the 25.0%and 46.4%in the control group(x^2 = 5.48,x^2 =4.25,P <0.05).Conclusion Transcranial drilling drainage combined with Ommaya tube is effective,safe for the treatment of cystic glioma.
