3日饮食日记在腹膜透析患者中的应用效果观察

摘　　要：目的观察3日饮食日记对腹膜透析患者血磷和营养状况的影响。方法选取四川省攀枝花煤业集团有限公司总医院收治的腹膜透析患者50例,在护理人员和营养科医生的指导下记录3日饮食日记,并根据患者摄入的营养成分制定相应的饮食治疗方案。随访6个月,在第1、3、6个月对3日饮食体积进行评估,比较其结果随着时间变化的整体差异,并比较患者血液磷、钾、钙等各项指标。采用SPSS 15.0进行数据分析处理,正态分布资料用(x±s)表示,非正态分布资料用中位数表示,采用t检验或者Wilcoxon配对检验。采用重复测量的方差分析比较随着时间变化的整体差异,P<0.05为差异有统计学意义。结果患者通过3日饮食日记,磷摄入量明显下降,随访的第1个月(771.57±195.04)mg/d和第6个月(696.30±187.81)mg/d相比,差异有统计学意义(P<0.05);血浆白蛋白明显上升,第1个月(36.78±4.08)g/L与第6个月(39.10±3.58)g/L相比,差异有统计学意义(P<0.05);血磷浓度明显下降,第1个月(2.04±0.52)mmol/L与第6个月(1.70±0.40)mmol/L相比,差异有统计学意义(P<0.05)。结论3日饮食日记可以有效改善腹膜透析患者的营养状况和降低血磷浓度。Objective To observe the effects of a three—days diet diary intervention on levels of serum inorganic phosphorus and nutrition intakes amongpatients withperitoneal dialysis treatment.Methods There were 50 patients underwent peritoneal dialysis included in the study.The patients wrote threeday diaries,under the guidance of nurses and nutrition physicians,to record their diary plans.Diary plans were adjusted to patient's prior diary intakes.Patientswere followed for 6 months.The serum electrolyte and nutrition status were measured after 1,3,6 months of the intervention.SPSS 15.0 was used to conduct the analysis.Normal distributed variablesweredescribedusing mean ± SD,and skewed variables were described using median.Pair t—test or Wilcoxon rank—sum test was used to compare between paris.Repeated Measures ANOVA was used to compare the changes with time.P<0.05 was considered as statistical significant.Results Theaverage level of serum inorganic phosphorus decreased with time(P<0.05),and it was 2.04+0.52 mmol/L after one month and 1.70+0.40 mmol/L after 6months.Theaverage level of albumin increased with time(P<0.05),and it was 36.78+4.08 g/L after one month and 39.10+3.58 g/L after 6 months.Conclusions Diet diary is effectiveto improve the nutrition and to decrease serum inorganic phosphorus in patients using peritoneal dialysis treatment.
