摘 要:目的 :探讨新辅助化疗对乳腺癌多药耐药表型的影响。方法 :采用免疫组织化学方法检测 33例临床Ⅲ期乳腺浸润型导管癌的P 糖蛋白 (P gp)和拓扑异构酶Ⅱ (TopoⅡ )的表达情况 ,观察了与术前化疗、雌激素受体 (ER)、肿瘤组织学分级的关系。结果 :P gp阳性染色定位于细胞膜和细胞浆 ,33例标本中 ,P gp表达阳性率为 5 4 .5 % (18/ 33) ,其中术前未化疗组为 5 2 .4 % (11/ 2 1) ,术前化疗组为 5 8.3% (7/12 ) ,两者差异无统计学意义 (P =1.0 0 ) ;TopoⅡ阳性染色定位于细胞核 ,TopoⅡ表达阳性率为 5 4 .5 %(18/ 33) ,其中术前未化疗组为 5 7.1% (12 / 2 1) ,术前化疗组为 5 0 % (6 / 12 ) ,两者差异无统计学意义 (P=0 .731)。P gp、TopoⅡ表达与ER、肿瘤组织学分级及P gp和TopoⅡ表达之间无明显相关关系。 结论 :乳腺癌存在着内源性多药耐药 ,术前新辅助化疗对P gp和TopoⅡ表达不产生显著性影响。Objective: To investigate the impact of neoadjuvant chemotherapy on multidrug resistance phenotype of breast cancer.Methods:The study was performed to evaluate the expression of P-glycorotein(p-gp) and Topoisomerase Ⅱ (Topo Ⅱ) in 33 patients with stage Ⅲ infiltrating ductal breast cancer by immunohistochemistry.The relationship between the expression of p-gp and Topo Ⅱ and neoadjuvant chemotherapy,ER status,histological grading were analysed.Results: P-gp positive staining was found in the cytoplasm and membrane of tumor cells.The positive rate of P-gp in 33 patients was 54.5% (18/33),52.4%(11/21) in the group with neoadjuvant chemotherapy and 58.3%(7/12) in the group without neoadjuvant chemotherapy respectively.No statistical difference was seen between the two groups.Topo Ⅱ positive staining was found in the nucleus of tumor cell.The positive rate of Topo Ⅱ in 33 patients was 54.5% (18/33),57.1%(12/21) in the group with neoadjuvant chemotherapy and 50%(6/12) in the group without neoadjuvant chemotherapy respectively.No statistical difference was seen between the two groups.Statistical analysis didn′t indicate any significant relationship between the expression of P-gp and ER status,histological grade.Nor did the expression of Topo Ⅱ and ER status,histological grade,or the expression of P-gp and Topo Ⅱ.Conclusion: There is intrinic multidrug resistance in breast cancer,and there is no significant impact of neoadjuvant chemotherapy on the expression of P-gp and Topo Ⅱ in stage Ⅲ infiltrating ductal breast cancer.
