颏下皮瓣在软腭贯通缺损一期修复中的应用

摘　　要：目的　探讨带血管蒂颏下皮瓣在软腭癌切除后软腭贯通缺损修复中的应用。　方法　软腭癌切除后致软腭部分或全部贯通缺损 10例 ,用颏下皮瓣一期修复。　结果　 6例皮瓣全部成活 ,2例皮瓣远端部分裂开坏死 ,2例完全坏死。　结论　颏下皮瓣在修复软腭缺损中 ,具有距离近 ,供瓣区隐蔽 ,厚薄适中 ,血供可靠 ,操作方便等优点 ,术后形态恢复较满意 ,是修复软腭缺损的一个理想选择。术中保留颈外静脉进入皮瓣的变异分支有利于保证皮瓣的静脉回流 ;Objective To explore the application of the pedicled submental flaps in reconstruction of the soft palate defect after the soft palate carcinoma resection. Methods 10 cases of part or total soft palate defect got their defects reconstructed with the pedicled submental flaps after the soft palate carcinoma resection. Results Six submental flaps were alive, 2 partly splitted at the end and another 2 got necrosis. Conclusions Because of its short distance, concealed area, fitting thickness, reliable blood supply and convenient operation and satisfactory reconstruction, the pedicled submental flaps is the ideal choice in repairing the soft palate defect. The successful key point is to manage the nude side of the flaps near the cavity of the nose well.
