抗抑郁治疗对脑卒中后抑郁患者神经功能康复的影响

摘　　要：目的　探讨抗抑郁治疗对脑卒中后抑郁患者神经功能康复的影响。方法　对 5 7例脑卒中患者随机分为两组 ,赛乐特治疗组 2 9例 ,对照组 2 8例 ,观察治疗前进行神经功能缺损程度评分 ,生活自理程度评分及汉密尔顿抑郁量表HAMD评分 ,对照组常规治疗原发病及康复训练 ,治疗组在此基础上同时合并使用赛乐特 2 0mg/d ,各组患者观察治疗 ,并随访至 1 2周末。在治疗后 2周、4周、8周、1 2周末复查上述各项指标。结果　两组患者治疗前抑郁量表HAMD评分、神经功能缺损程度评分及生活自理程度评分经t检验P >0 0 5 ,无显著差异 ;治疗后 2周、4周、8周治疗组与对照组以上各种评分比较经t检验P <0 0 1 ,有高度显著性差异。两组患者抑郁疗效比较 :治疗组总有效率 86 2 % ,对照组总有效率 46 43% ,经X2 检验P <0 0 5 ,有显著性差异。两组患者神经功能疗效比较 :治疗组总有效率 89 66% ,对照组5 7 1 4% ,经X2 检验P <0 0 5 ,有显著性差异。结论　对脑卒中后抑郁患者用赛乐特抗抑郁治疗不仅能改善患者的抑郁症状而且有利于神经功能的康复 ,而且副作用小 。Objective To investigate the effects of anti-depression theraputics forneurological function recovery on post-stroke depression Methods Fifty seven patients with post stroke depression were divided randomly into two groups,Seroxat group(29 cases) and control group(28 cases)control group only received the general treatment and convalescent training,but seroxat group did the general treatment ,convalescent training and seroxat 20 mg daily Before treatment and after 2,4,8 and 12 weeks, Neurological function defection scale, Self life scale and HAMD scale was evaluated respectively Results there were not difference between seroxat group and control group in neurological function defection scale,Self-life scale and HAMD scale before treatment, but there were significant differences at 2,4,8 and 12 weeks after treatment respectively﹙ P <0 01) The patients with improved depressive symptom were 86 2% in Seroxat group and 46 43% in control group( P <0 05 )Thepatients with improved neurological function defection were 89 66% in Seroxat group and 57 14% in control group( P <0 05) Conclusion Anti depression therapeutics with Seroxat on post stroke depression not only improve depressive symptom but also benefit to neurological function defection,and have little side effects and good compliance
