摘　　要：目的　探讨子宫动脉栓塞术 (TUAE)治疗子宫肌瘤的价值。方法　经B超和 (或 )CT确诊为子宫肌瘤的 2 4例患者作介入治疗 ,采用Seldinger技术 ,经双侧子宫动脉注入聚乙烯醇 (PVA)颗粒和高压消毒明胶海绵颗粒充填子宫病灶进行栓塞。结果　 2 4例行 2 5例次双侧子宫动脉栓塞 ,随访 1～ 1 2个月 ,8例失访。术后 1个月所有病例月经量明显减少 ,与介入前进行比较减少均 >50 % ;1 0 0 %病例继发性贫血症状得到明显改善 ;2 2例病例局部症状消失或减轻。B超随访术后 3～ 6个月 ,瘤体体积缩小约在 30 %～ 80 % 2 0例 ,2例缩小 >80 % ,2例缩小 <30 %。 1例随访 9个月后复发 ,再次行栓塞治疗 ,近期效果较好。其余病例随访 1～ 1 2个月未见明显复发征象。全部患者术后均有不同程度下腹痛 ,部分病例有恶心、呕吐 ,无明显发热 ,未发生严重并发症。结论　TUAE是治疗子宫肌瘤的安全有效、简便、创伤小。Objective To assess clinical value of transca theter uterine arterial embolization(TUAE) on uterine fibroids. Methods 24 patients with uterine fibroids diagnosed by B ultrasound or CT were treated with TUAE. By using Seldinger's method, PVA was injected in bilateral uterine arteries, and high pressure sterilized gel foam particles were used to embolize the arteries. Results 25 times of bilateral uterine arterial embolization were performed in 24 patients. The improvement in symptoms and uterine size changes was followed up in 1～12 months. After interventional treatment in 1 month the volume of menstruation decreased more than 50%, anaemia symptoms were improved significantly in 100% patients. Local symptoms in some patients were relieved or completely disappeared. By sonograph ical follow-up in 3～6 months, the volume of uterine focus decreased from 30% to 80% in 20 follow-up cases, more than 80% in 2 cases, less than 30% in 2 cases. 1 case recovered after interventional treatment in 9 months and was re-operated. The other had not recovered. All patients had hypogastric pain. Some cases had felt nausea and vomited, but had not manifested fever and other intense complications. Conclusion TUAE is safe, effective, convenient for interventional treatment of uterine fibroids.
