Herbst矫治器治疗生长期儿童的Helkimo指数分析

摘　　要：目的 :探讨Herbst矫治器治疗对颞下颌关节 (TMJ)的影响 .方法 :我们采用国产Herbst矫治器对 17例生长发育期安氏II1下颌后缩儿童进行临床矫治 ,分别于戴Herbst矫治器前 (T1) ,戴Herbst矫治器后 1wk(T2 ) ,Herbst矫治器拆除后 1wk(T3)进行常规临床检查 ,做Helkimo指数分析 .结果 :T1期主诉症状指数Ai0为 13/ 17,AiI为 4 / 17;T2期Ai0为 3/ 17,AiI为 11/ 17,AiII为 3/ 17;T3期Ai0为 15 / 17,AiI为2 / 17.即T1与T2期 ,T2与T3期有显著性差异 ,T1与T3期无显著性差异 .而T1期临床症状指数Di0为 9/ 17,DiI为 8/17;T2期DiII为 10 0 % ;T3期Di0为 14 / 17,DiI为 3/ 17.即T1与T2期 ,T2与T3期 ,T1与T3期均差别显著 .结论 :在Herbst矫治器治疗初期 ,Helkimo指数有所增加 ,但矫治结束后 ,关节症状消失 .Herbst矫治器治疗一般不会引起颞下颌关节紊乱 (TMJD) .AIM: To investigate the influence of Herbst appliance therapy on temporomandibular joints (TMJ). METHODS: 17 of Class II Division 1 malocclusion cases were treated with the domestic Herbst appliance. Helkimo index evaluation was made before wearing Herbst appliance (T1), a week after wearing Herbst appliance (T2) and after debonding Herbst appliance (T3). RESULTS: During T1 stage, 76.5% and 58.8% patients were free of anamnestic and clinical dysfunction respectively, and 23.5% and 41.2% patients had a mild anamnestic and clinical dysfunction. During T2 stage, 17.6% patients were free of anamnestic dysfunction, and 64.7% and 17.6% patients had a mild and a severe anamnestic dysfunction respectively, but all of the patients had a severe clinical dysfunction. During T3 stage, 88.2% and 82.4% patients were free of anamnestic and clinical dysfunction, and 11.7% and 17.6% patients had a mild anamnestic and clinical dysfunction. The difference of Ai and Di was significant between T1 and T2 stage. So was the case between T2 and T3 stage. But between T1 and T3 stage, the difference was insignificant. CONCLUSION: At the beginning of Herbst appliance therapy, Helkimo index increases. After therapy, symptoms of temporomandibular joint disorder (TMJD) are relieved. It seems that Herbst appliance therapy may not worsen TMJD or lead to TMJD. However, further study is needed for the long term effects of Herbst appliance.
