低位直肠癌保肛手术中器械吻合的应用

摘　　要：目的　探讨在器械吻合低位直肠癌保肛术中减小创伤、提高疗效的临床应用价值。方法　自 1992年 5月～ 2 0 0 0年 12月间应用管状消化道吻合器行直结肠端端、端侧吻合治疗 131例低位直肠癌患者 ,对治疗结果进行总结。结果　 131例中男性 6 0例 ,女性 71例 ,年龄 2 9～ 78岁 ,平均 5 4岁。根治性切除 10 9例 ,姑息性切除 2 2例。肿瘤下缘距肛门≤ 6cm者 5 9例 ,≤ 8cm者 72例。用GF -Ⅰ型 31号吻合器吻合 12例 ,34号吻合器吻合 119例。肿瘤切除后剩余直肠端长 4～ 10cm。吻合口狭窄 3例 ,发生率 2 .3% ;吻合口漏 3例 ,发生率 2 .3% ,均经保守治疗治愈。根治性切除 10 9例中术后盆腔复发 6例 ,发生率 4 .6 %。结论　采用吻合器行直结肠端端、端侧骶前吻合保肛术治疗低位直肠癌具有创伤较小 ,操作简单安全 ,吻合成功率高 ,并发症少 ,患者术后生活质量较高等优点 ,值得临床推广应用。Objective:To study the clinical application of stapling device in Anus-preserving operation of low rectal cancer. Methods:131 cases of low rectal cancer were treated surgically with The GF-Ⅰ straight-tube stapling device were retrospectively analyzed.Results:Sixty patients were male and 71 were female, age from 29～78 years old, with an average of 54 years old. One hundred and nine patients had the radical resection and 22 had a palliative operation. The lower margin of the tumor was less then 6cm in 59 patients and less then 8em in 72 patients. The remain rectum was about 1～4 cm long. cases of anastomosis sterosis (2.3%) and 3 cases leak of anastomosis (2.3%) were founded, all these were cured with no-operation. 5 cases of pelvic cavity recurrence (4.6%) were found in the radical resection group.Conclusions:The indication of the operation preserving the anus of rectal cancer at lower segments was extended by using the stapling device and it is simple, safety and inexpensive wit lower complication and higher quality of life.
