囊外摘除治疗西藏地区白内障手术体会

摘　　要：目的 探讨高海拔地区白内障囊外摘除术的效果及安全性。方法 对516例(535眼)白内障患者施行囊外摘除联合人工晶体植入手术,观察手术前后裸眼视力,以Placido盘测定角膜散光,记录并发症。结果 术后第3天平均裸眼视力为0.48±0.27,角膜散光0级者158眼,1级者209眼,2级者130眼,3级者38眼。术中后囊破裂14眼(2.6％),虹膜反复脱出60眼(11.2％),虹膜根部离断4眼(0.7％)。术后角膜水肿67眼(12.5％),虹膜伤口嵌顿12眼(2.2％),一过性眼压升高34眼(6.4％),瞳孔变形27眼(5.0％)。结论 高海拔地区白内障囊外摘除手术效果好,并发症较少,视力恢复较快。Objective To assess the effectiveness and safty of ECCE(extra-capsular cataract extra -tion) in treatment of cataract on high altitude. Methods ECCE plus intraocular lens implantation were performed on 516 cases(535 eyes) . The naked visual acuity, corneal astigmatism and complications were observed postoperatively. Results The third day postoperatively, the average naked visual acuity was 0.48 ±0.27, the corneal astigmatism was grade 0 in 158 eyes, grade 1 in 209 eyes, grade 2 in 130 eyes,grade 3 in 38 eyes. During operation, the posterior capsular was ruptured in 14 eyes(2.6%), the iris protruded in 60 eyes( 11.2%), the iris root was breaken in 4 eyes(0.7% ) . Postoperatively, cornea edema was found in 67 eyes( 12.5% ), iris embeded was found in 12 eyes(2.2% ),transient high intraocular pressure was found in 34 eyes(6.4% ), pupil deformation was found in 27 eyes(5.0% ). Conclusion ECCE is effective in treatment of cataract on high altitude, the complications are few, and the patients recover more fast.
