应用自体肺移植技术治疗Ⅲ期上叶中心型肺癌

摘　　要：目的　探讨应用自体肺移植技术治疗Ⅲ期上叶中心型肺癌的可行性。方法　 4例作双袖状左上肺叶切除 ,因肺动脉切除过长 ,肺动脉吻合张力过大 ,遂切断下肺静脉 ,肺短时间离体后 ,将下肺静脉移植在上肺静脉残端。结果　随访 2～ 3年 ,3例患者已分别无瘤存活 3 8、2 8、2 5个月 ,生活质量良好。 1例术后 40h发生肺静脉血栓 ,被迫行 2次手术摘除移植的下肺。结论　对心肺功能不能耐受全肺切除的Ⅲ期上叶中心型肺癌患者 ,可以考虑应用自体肺移植技术。自体肺移植是一种能保全肺组织的肺癌根治术式。Objective: To explore the feasibility of lobar replantation i n the treatment of stage Ⅲ central lung cancer of the upper lobar. Met hods: Four patients underwent double-sleeve left upper lobectomy. Sinc e the length of resected pulmonary artery involved by tumor was too long to perf orm tension-free anastomosis, the inferior pulmonary vein to the proximal stump of the supperior pulmonary vein was transplanted, so the middle and lower lobes were preserved. Results: Followed up to Dec 2003, 3 patients had been alive for 38, 28 and 25 months separately with a good living quality. O ne case received resection of the replanted lung for pulmonary vein thrombus aft er 40 h postoperatively. Conclusion: Lobar replantation is an alte rnative procedure of parenchyma-sparing in patients with stage Ⅲ central lung cancer of the upper lobe whose cardiopulmonary function is too poor to undergo p neumonectomy.
