60例FEVR患眼B型超声图像分析

摘　　要：目的分析B型超声检查在家族性渗出性玻璃体视网膜病变(FEVR)诊断中的价值。方法回顾性分析60例FEVR患儿眼B型超声形态学特点,并与其眼底检查结果进行对照分析。结果双眼累及53例,单眼7例。超声图像正常14眼;全视网膜脱离54眼;局限视网膜脱离3眼;镰状视网膜脱离26眼(占22%),超声纵行扫查呈现连于视盘和周边球壁前的新月形膜样回声,其中24眼位于颞侧;单纯玻璃体增殖条索或视盘颞侧球壁回声增厚16眼;单纯玻璃体点状弱回声7眼。超声检查和眼底检查对疾病分级结果对应良好(符合率为82.5%,Kappa值为0.92)。结论除单纯周边血管异常外,B型超声能良好反映FEVR患眼的各种玻璃体视网膜病变。虽无特异性,但镰状视网膜脱离尤其发生在双眼颞侧时应高度怀疑FEVR可能。对侧眼玻璃体少量混浊提示存在周边无血管区及渗漏可能。Objective To analyze the value of B-scan ultrasonography in diagnosis of familial exudative vitreoretinopathy(FEVR)eyes.Methods We retrospectively analyzed the morphological characteristics of B-mode ultrasonography on eyes of 60 FEVR patients,and compared to their fundus findings.Results 53 binocular FEVR,and 7monocular.Ultrasonography showed 14 normal eyes;54eyes with total retinal detachment;3eyes with local retinal detachment;26eyes with falciform retinal detachment(22%),24 of which occurred temperally,demonstrating a crescent-like membrane sheet in front of the global wall from disc to periphery by longitudinal scan;16eyes with vitreous bands or thickening of global wall temperal to the disc as the only finding;7eyes with only mild hypoechoic points in vitreous.The severity classified by ultrasonography and fundus examination showed high correlation with a compliance rate of 82.5%,and a Kappa value of 0.92.Conclusions Except simple peripheral vascular abnormality,ultrasonography is able to show corresponding positive findings for a variety of vitreoretinal lesions of FEVR.Although not specific,falciform retinal detachment especially occurrs temperally of both eyes may highly suggest the diagnosis of FEVR.Mild vitreous opacification of the contralateral eye may suggest the presence of possible peripheral avascular zone with leakage.
