下颌水平埋伏阻生智齿高速涡轮钻增隙的临床应用效果

摘　　要：目的探讨下颌水平埋伏阻生智齿高速涡轮钻增隙的临床应用效果。方法随机抽样法选取2017年5月至2018年5月本院72例下颌水平埋伏阻生智齿患者(72颗患牙),随机信封法分为研究组(n=36,行高速涡轮钻增隙)与对照组(n=36,行传统拔牙术)。比较治疗效果。结果①研究组手术时间短于对照组(t=10.790,P=0.000),术后24 h VAS评分低于对照组(t=12.096,P=0.000);②研究组张口受限轻度率高于对照组(χ~2=4.430,P=0.035),重度率低于对照组(χ~2=4.545,P=0.033);③研究组术后不良事件少于对照组(χ~2=4.571,P=0.032)。结论高速涡轮钻增隙在下颌水平埋伏阻生智齿治疗中的应用,具有手术时间短、疼痛程度轻、术后不良事件少等特点,且可促使开口度尽快恢复。Objective To explore the clinical application effects of gap increase with high speed turbine drill in the extraction of horizontally impacted mandibular wisdom teeth.Methods 72 cases(72 horizontally impacted mandibular wisdom teeth)were selected for the study by random sampling method from the patients with horizontally impacted mandibular wisdom tooth who were treated in Xianghe County People’s Hospital between May 2017 and May 2018.They were divided into an observation group(n=36)was adopted and a control group(n=36)evenly by random envelope method.Gap increase with a high speed turbine drill and traditional tooth extraction were applied in the two groups respectively.The effectiveness was compared between the two groups.Results Firstly,the operation duration of the patients in the observation group was shorter than that in the control group(t=10.790,P=0.000).The VAS score 24 hours after operation in the observation group was lower than that in the control group(t=12.096,P=0.000).Secondly,compared with the control group,the observation group had a higher rate of light limitation of mouth opening(χ~2=4.430,P=0.035)and a lower rate of serious limitation of mouth opening(χ~2=4.545,P=0.033).Thirdly,there were fewer postoperative adverse events in the observation group than in the control group(χ~2=4.571,P=0.032).Conclusion Compared with traditional tooth extraction,gap increase with high-speed turbine drill in extraction of horizontally impacted mandibular wisdom teeth enjoys the advantages such as shorting operation time,reducing pain,involving fewer postoperative adverse events,and promoting the recovery of mouth opening.
