肠内和肠外营养对上消化道肿瘤术后肠瘘患者的肠道保护作用

摘　　要：目的　研究肠内和肠外营养对上消化道肿瘤术后肠瘘患者的肠道功能保护作用 ,提高其营养支持效率。方法　 2 8例实验对象 ,随机分为肠内和肠外营养支持两组 ,采用前瞻性随机对照研究。结果　肠内营养支持可降低循环内毒素水平 ,减轻肠道缺血再灌注损伤。降低血循环中炎症介质TNF和IL 8的水平。结论　肠内营养可有效保护上消化道肠瘘患者的肠道功能 ,削弱、阻滞“内毒素 炎症介质 肠粘膜损害”这一恶性循环。Objective To study the protective effects of gut by the nutrient sustain on upper digestive tract tumour patients with enteral fistula after operation. Methods Twenty eight cases were randomly divided into enteral nutrition group (EN) and parenteral nutrition group (PN). The method is prospective study . Results The results showed that the EN group reduced the reperfusion injury of the gut, and decreased the levels of endotoxin,TNF and IL 8 in blood stream. Conclusion Enteral nutrition could preserve the function of gut to the upper digestive tract tumour patients with enteral fistula after operation, and blunt the chain reaction of 'endotoxin inflammatory mediator gut mucosa injury'.
