综合康复训练治疗偏瘫后肩痛的临床观察

摘　　要：目的研究综合康复治疗对脑卒中偏瘫后肩痛的疗效观察。方法将80例脑卒中后肩痛患者随机分为两组,对照组40例,采用运动疗法训练;治疗组40例,在对照组的基础上加用超短波、温热磁及中药熏洗进行治疗,在治疗前和治疗4周后,通过疼痛的视觉模拟评分法(VAS)、Fugl-Meyer(FMA)上肢功能评估量法对两组患者患肩的疼痛程度及上肢运动功能进行评估。结果治疗4周后,治疗组在减轻肩痛程度上明显优于对照组(P<0.05),FMA上肢功能评分显著高于治疗前(P<0.05),亦优于对照组(P<0.05)。治疗组总有效率为97.50%,明显高于对照组的87.50%(P<0.05)。结论综合康复训练治疗脑卒中后偏瘫患者肩痛疗效满意。Objective: To observe the effect of comprehensive rehabilitation treatment on the shoulder pain in patient with hemiplegia. Methods: 80 patients with post-stroke hemiplegia shoulder pain were randomly divided into control group and treatment group,40 cases in the control group were treated with exercise therapy;40 cases in the control group were treated with ultrashort wave,thermal magnetic treatment and herbal fumigation based on the use of control group. The Visual Analogue Scale(VAS) scores and the Fugl-Meyer(FAM) upper limb function score were used to evalute patients suffering from shoulder pain and movement function of upper limbs from two groups before and 4 weeks after treatment. Results: After 4 weeks,the treatment group was significantly better than the control group in the degree of reducing the shoulder pain(P < 0.05). The Fugl-Meyer upper limb function score was significantly higher than that before treatment(P < 0.05),especially in the treatment group(P <0.05). The total effective rate in the treatment group was 97.5%,which was significantly higher the total effective rate in the control group(87.5%). Conclusion: Comprehensive rehabilitation has a satisfactory effect on treating patients with hemiplegia shoulder pain after stroke.
