脑卒中患者行为记忆能力的临床分析

摘　　要：目的:探讨脑卒中患者行为记忆能力的临床特点,分析其相关影响因素,为临床康复训练计划的制定提供依据。方法:应用Rivermead行为记忆检查(RBMT)对67例脑卒中患者及58例正常人进行评测,同时收集包括病史、既往史、病灶情况等相关因素的临床资料。结果:①脑卒中组与正常对照组比较,在RBMT总分与7个分项目上脑卒中组明显下降(P<0.05);②脑出血与脑梗死患者的行为记忆能力差异有显著性(P<0.05);病灶部位在皮层的患者其RBMT总分明显低于病灶部位在皮层下的患者(P<0.01);单病灶与多病灶患者比较两者有非常显著性差异(P<0.001);病程越长、发病次数越多,患者的行为记忆能力越差(P<0.01)。但病灶位于左侧、右侧或病灶累及双侧的患者相比,三者之间差异无显著性(P>0.05)。③在照片再认及即刻、延迟故事回忆三个分项目上,左侧病灶者与右侧病灶者比较,两者差异有显著性(P<0.05)。结论:脑卒中患者的行为记忆能力受到损害;其病变的性质、病灶部位和数量、病程及发病次数均对患者的行为记忆能力有显著影响。Objective:To approach clinical characteristics of behavioral memory function for stroke patients and analyze its influence factors.Method:67 cases of stroke and 58 cases controlled normal were assessed by Rivermead behavioral memory test. Meanwhile, relative clinical information such as medical record?past history?focus condition were collected.Result:①The total score and 7 items score of RBMT were markedly decreased on stroke group.②There were remarkable differences on behavioral memory ability between cerebral hemorrhage and cerebral infarction patients. The patients of focus at cortex had less behavioral memory ability than the patients of focus at sub-cortex. There were much remarkable differences between the patients of multi-focus and single focus.③There were obvious differences on 3 items of the photo recognition?immediate and delayed story memory between the patients of left hemisphere and right hemisphere damage. Conclusion: The behavioral memory function of stroke group was damaged.
