摘  要:目的为研究角蛋白克隆抗体免疫组织化学法在检测淋巴结微转移灶上的意义。方法采用三种角蛋白单克隆抗体AE1,AE3、AE1/AE3,对我院47例大肠癌术后患者331个淋巴结用免疫组织化学法重新检查。结果36个淋巴结存在微转移灶(11.9%),Duke'sC期患者微转移灶检出率高于Duke'B期患者(P<0.05)。随访显示有微转移灶Duke'B期病人肿瘤复发、远处扩散转移均较无微转移病人高(P<0.05)。结论即使是组织学检查结果为阴性的淋巴结,也很有存在微转移的可能性。免疫组织化学法栓测单克隆抗角蛋白抗体,可详细阐明临床分期,并可提供大肠癌治疗的指征。Objective To study the significance of detection of lymp h node micrometastasis by immunohistochemistry method with monclonal anti-karati n antibody in patirets with colorectal cancer.Methods We examined 331 lymph ne des obtained from 47 colorectal cancer patients who underwent a curative operati on and diagnosed by using immuohistochemistry SP techniques.Monoclonal mouse ant i-keratin antibodies (AE1,AE1/AE3,AE3) were used as the primary reagents to iden tify tumor cells both in tumor tissues and lymph nodes. Results A total of 36 micrometastases among 331 lymph nodes (11.9%) were found. Duke's C staging pa tients had more lymph nodes metastasis than in Duke's B staging patients ( P<0.05).Postoperative follow-up revealed that recurrence rate was higher,and the latent period of distant metastasis was much shorter in patients with occult mi crometastasis than in those without.Conclusion Even in histologically negativ e lymph nodes,there is a considerable possibility that micrometastasis may exist .The immunohistochemical detection of monoclonal anti-keration antibody can expl icate clinical staging,and offer indication for colorectal cancer therapy.
