摘  要:目的 探讨缺氧诱导因子-1α(HIF-1α)、热休克蛋白70(HSP70)在脑出血灶周脑组织中的表达及其意义。方法 应用免疫组织化学技术检测37例脑出血灶周脑组织中HIF-1α、HSP70的表达。结果 37例脑出血灶周脑组织中HIF-1α阳性率为40.5%,其中出血量>60 ml者HIF-1α阳性率为88.9%,与其他出血量者(30-45 ml,46-60 ml)比较差异有显著性意义(P<0.05)。HIF-1α表达与血肿作用时间明显相关,随着出血后手术时间的推迟而增加(P<0.05)。HIF-1与HSP70的表达呈正相关(n=15,r=0.82,P<0.01)。结论 脑出血后HIF-1α的表达与出血量及血肿作用时间相关,为判断脑出血灶周缺血半暗带的存在提供了依据;HSP70有神经细胞保护及抑制凋亡的作用。Objective To evaluate the expression of hypoxia inducible factor-1α (HIF-1α) and heat shock protein 70 (HSP70) in perihematoma with intracerebral hemorrhage, and to explore their possible roles after cerebral hemorrhage. Methods The expression levels of HIF-1α and HSP70 were assessed by inmunohistochemistry straining. Results In 37 cases of hypertensive hemorrhage, positive rate for HIF-la was 40. 5 %. The positive rate in the patients with the volume of blood loss >60 ml (88. 9 %) was significantly higher than those with the volume of blood loss being 30-45 ml or 46-60 ml. The HIF-1α expression was closely related with the choice of the operating opportunity and increased with the delay of the operating time (P<0. 05). There were a positive correlation between HIF-1α labeling index and HSP70 index (n=15, r=0. 82, P<0. 01). Conclusion The expression of HIF-1αwas closely related with the choice of operating opportunity and the volume of blood loss, which provided a foundation that the ische-mic penumbra existed in the perihematoma. HSP70 could protect nerve cells and inhibit apoptosis.
