摘　　要：目的 探讨局部应用胰岛素样生长因子(IGF-1)对面神经损伤的修复作用。方法 将20只成年新西兰兔面神经上颊支切除5mm,在断端间置入硅胶再生室。左右侧随机分组,采用自身对照,在实验耳室内注入IGF-1.对照耳用生理盐水,术后4周和8周进行电生理学、组织病理学观察和形态学定量分析。结果 术后4周,两组电刺激面神经很少能引发面肌兴奋.有髓轴索计数.轴索直径和面积t检验.两组差异有显著性意义(P<0.05)。术后8周实验耳运动神经传导速度(MCV)明显短于对照耳,有髓轴索计数,轴索直径和面积实验耳大于对照耳(P<0.05)。结论局部应用IGF-1可促进面神经损伤的修复。Objective To investigate the effects of insulin-like growth factor-1 (IGF-1) on the regeneration of facial nerve following injury to it. Methods 5 mm of superbuccal branch of facial nerve was resected in both sides at first among 20 New Zealand rabbits and a silicone tube .(ST) was placed here to connect the two ends of resected nerve in each side. Then, self random group-dividing method was used to each of the experimental animals with one side in treating group(TG) and the other in controlling one(CG). Following this procedure, IGF-1 was injected into ST of TG and normal saline into ST of CG. By the end of 4th and 8th weeks of experiment, electrophysiological tests, histopathological observation and quantitative morphological determination were carried out among them respectively. Results By the end of 4th week, significant differences had been shown in the average number of myelinated axons and the area and diameter of cross-sectional axons (P<0. 05) between the two groups while there was little exciting reponse seen following electric stimulating to the facial nerve. By the end of 8th week, the motor nerve conduction velocity (MCV) of facial muscle excitement was 38. 7 ± 4. 4m/s in TG and 31. 345. 6m/s in CG, with significant difference between them(P<0. 05) .Furthermore, the average number of myelinated axons, cross-sectonal area of axons and diameter of them were 1955 ± 471. 2. 21 ± 0. 33 and 3. 19 ± 0. 41 in TG respectively, significantly higher than that of CG (being 1278 ± 396,1. 79 ± 0. 35 and 2. 56 ± 0. 58 respectively), with the P values all less than 0. 05. Conclusion Local application of IGF-1 can promote the regeneration of facial nerve following in jury to in adult New Zealand rabbits.
