局部晚期非小细胞肺癌超分割与常规放疗结合化疗对照

摘　　要：[目的]比较超分割与常规放疗结合化疗综合治疗局部晚期非小细胞肺癌的疗效和放疗毒副反应。[方法]64例经病理证实ⅢA、ⅢB期非小细胞肺癌患者(排除恶性胸腔积液者),随机分成A组:超分割放疗联合MVP(丝裂霉素,MMC;长春地辛,VDS;顺铂,DDP)化疗组;B组:常规放疗联合MVP化疗组。各组病人均给予4～6个疗程化疗,放疗在化疗2个疗程后实施,超分割治疗组予以70Gy/60Fx、6w,常规放疗组予以(60～64)Gy/(30～32)Fx、(6～6.5)w。[结果]两组患者均耐受并完成治疗,近期有效率(CR+PR)分别是A组82.7%,B组71.5%(P>0.05),A组和B组的放射性食管炎、气管炎、肺炎发生率分别是79.3%、62.1%、31.0%和62.9%、57.1%、28.6%(P>0.05)。[结论]放化疗综合治疗局部晚期非小细胞肺癌中超分割放疗较常规放疗近期有效率有改善,但无显著性差异,两种放疗的毒副作用相近。To compare the effects and acute radiation toxicities of hyperfractionated radiotherapy(HF)plus chemotherapy and conventional fractionated radiotherapy(CF)plus chemotherapy for local ad-vanced non-small cell lung cancer(NSCLC).Sixty-four pathologically provedⅢA,ⅢB stage NSCLC patients were divided randomly into two groups:1.group A,HF plus MVP(mitomycin C,MMC;vin-desine,VDS;cisplatin,DDP)chemotherapy,patients were irradiated1.2Gy/f bid,with a more than6hours in-terval,10fraction per week,to a total dose of70Gy.2.group B CF plus MVP chemotherapy,patients were irradiated2Gy/f,5times a week,to the total dose of60Gy^64Gy.The chemotherapy was performed for4~6courses and the radiotherapy was performed after the second course of chemotherapy.Both of the two groups of patients completed their treatment course.The overall tumor response rates were82.7%in group A and71.5%in group B(P>0.05).The rates of acute radiation esophagitis,bronchitis and pneumonitis were79.3%,62.1%,31.0%in group A and62.9%,57.1%,28.6%in group B,respectively(P>0.05).[Conclusions]HF plus chemotherapy seems to improve the effect of local advanced non-small cell lung cancer,but without significant difference compared to conventional fractionated radiotherapy plus chemotherapy.Acute radiation toxicities is similar in two groups.
