摘 要:目的:比较正常人胸腺和重症肌无力胸腺树突状细胞(thymicdendriticcells,TDC)体外培养的差异,为研究胸腺树突细胞在重症肌无力抗原提呈中作用奠定基础。方法:正常成人、幼儿和重症肌无力(MyastheniaGravis,MG)胸腺组织,采用梯度离心法获得胸腺低密度细胞,经粒-巨噬细胞集落刺激因子(granulocyte-macrophagecolony-stimulatingfactor,GM-CSF)和干细胞因子(stemcellsfactors,SCF)的诱导,使TDC前体细胞生长为成熟TDC,比较正常胸腺组织与MG胸腺组织中TDC细胞生长状况。结果:所有胸腺组织均能诱导TDC,但不同来源胸腺组织中TDC生长状况明显不同,其中正常幼儿有一定程度TDC细胞生长,与成人比较具有显著差异;胸腺增生与胸腺萎缩组TDC细胞数量均明显增加,与正常幼儿组比较具有显著差异;胸腺瘤组TDC数量与正常幼儿组相似,但比正常成人组明显增加。结论:MG患者切除之胸腺组织病理学差异非常明显,但患者TDC细胞数量均明显增加,说明三者在激发自身免疫反应方面可能具有相似之处,其TDC是否具有特异性AchR抗原提呈功能,尚需进一步研究。AIM:To investigate the difference between in vitro culture of thymic dendritic cells(TDCs) in normal and myasthenia gravis thymus in order to establish the ground work for the research of TDC physiologic function and antigen presenting in myasthenia gravis(MG). METHODS:The fresh thymic tissue from normal adults and children and myasthenia gravis patients was dispersed mechanically in Hank's balanced salt solution,and the low density cells were obtained by gradient centrifugation and cultured with granulocyte macrophage colony stimulating factor(GM CSF) and stem cell factor(SCF) to make the precursor TDC mature.The growth status of TDC from normal and MG thymic tissue was compared.The generated cells were identified by ultrastructural,phenotypic and functional methods. RESULTS:The TDCs were obtained from all fresh thymic tissue,but there were many difference in growth status between normal and MG.The TDC in children grew certainly,which was significantly different from that in adults;The quantity of TDCs in the thymic proliferation and atrophy group was obviously increased,which was significantly different from that in the normal children group.The quantity of TDCs was similar between the thymic tumor group and normal children group,but was higher than that in the normal adult group. CONCLUSION:There are many differences in histopathological changes of myasthenia gravis thymus,but the quantity of TDCs apparently increases in hyperplasia,atrophy and thymoma of thymus.It indicates that there is maybe similar in autoimmune reaction,but the specific antigen presenting to AchR function of the thymic dendritic cells from myasthenia gravis need more study.
