摘  要:目的 研究乳腺癌中神经内分泌分化情况 ,及其与nm 2 3、pS2基因及淋巴结转移的关系。 方法 采用免疫组化S P法 ,对 5 8例乳腺癌手术标本用神经内分泌抗体NSE、Sy、CgA进行测定 ,观察其表达情况 ;同时进行nm 2 3、pS2基因检测并与淋巴结转移情况共同分析。结果  5 8例乳腺癌患者中NSE、Sy、CgA 3种抗体同时表达阳性者 14例 ,占 2 4% ,电镜下观察发现肿瘤细胞中有神经内分泌颗粒。NE阳性者nm 2 3、pS2表达水平均低于NE阴性者 ,而NSE阳性者淋巴结转移率 ( 5 7% ,8/14 )较NE阴性者 ( 2 7% ,12 /4 4)高 (P <0 .0 5 )。结论 乳腺癌伴神经内分泌分化是肿瘤异质性的表现 ,是乳腺癌的 1种特殊类型 ,具有与一般类型乳腺癌不完全一致的生物学行为 。Objective To study the expression of neuroendocrine(NE) antibody in breast cancer and its relationship with the prognostic factors of nm23,pS2 gene and the lymph node metastasis.Methods The expression of neuroendocrine index such as NSE,SY,CgAS-P was detected by immunohistochemical method and verified by electron microscopic in 58 cases of breast carcinoma.The nm23,pS2 gene were also detected and analysed with the state of axillary lymph node metastasis.Results The positive expression rate of NE,Sy,CgA antibody was 24%(14/58).There were NE dense granules in the cytoplasm of the tumor cells under electron microscopic.The expression of nm23 and pS2 protein was lower in patients with a positive expression of NE than in patients with a negative expression of NE.The incidence of lymph node metastasis was higher in patients with a positive expression of NSE than in patients with a negative expression of NSE(57% vs.27%,P<0.05).Conclusion Neuroendocrine differentiation in breast carcinomas is the heterogenicity of carcinoma originating from breast gland.It is a specific type with a different biological behavior from ordinary pathological type of breast carcinoma.So it is important to determine the postoperative treatment.
