心理并药物治疗与单纯药物治疗帕金森病伴发抑郁症患者的作用比较

摘　　要：目的:观察心理治疗合用药物及单用药物对帕金森病伴发抑郁的疗效。方法:62例帕金森病患者在治疗的同时,对伴发的抑郁症状加服帕罗西汀,并随机对其中32例配合心理治疗。结果:治疗第4周末两组汉密尔顿抑郁量表(HAMD)减分率与治疗前比较,差异均有非常显著性意义(F=230.2,89.42,P=0.000),但实验组在第1,2,4周末3个减分率犤(42.30±4.18)%,(57.26±5.63)%,(84.73±9.38)%犦均优于对照组犤(33.49±5.07)%,(49.13±5.90)%,(73.79±15.53)犦,差异有非常显著性意义(P<0.01)。第4周末实验组显效率为91%(29/32),对照组为67%(20/30),两组比较χ2=5.36,P=0.021。两组均未发现明显的不良反应。结论:心理治疗合用帕罗西汀对帕金森病伴发抑郁患者的疗效优于单用帕罗西汀。AIM:To observe the curative effects of psychological treatment plus drug and single drug therapy in the treatment of depression following Parkinson's disease(PD).METHODS:During the PD treatment,62 patients with depression were randomly divided into control group of 30 cases(paroxetine) and experiment group of 32 cases(psychotherapy plus paroxetine).RESULTS:Four weeks after treatment,both two groups had significant decrease in the scores of Hamilton rating scale for depression(HAMD) as compared with those before treatment(F=230.2,89.42,P=0.000).The HAMD scores in the experiment group were declined by(42.30±4.18)%,(57.26±5.63)%and(84.73±9.38)%at 1,2 and 4 weeks after treatment,all higher than those in the control group[(33.49±5.07)%,(49.13±5.90)%and (73.79±15.53)%respectively](P< 0.01). Marked improvement was found in 29 cases out of the experiment group(91%,29/33) and 20 of the control group(67%,20/30),with no significant difference between the two groups(χ2=5.36,P=0.021) .CONCLUSION:Psychotherapy plus paroxetine has better effect for PD patients with depression than single paroxetine.
