摘  要:目的 :对照研究CD34和CD1 0 5在胃癌微血管中的表达并探讨胃癌间质抗CD1 0 5抗体标记的微血管密度 (MVD)和血管内皮生长因子 (VEGF)表达与肿瘤浸润和转移的关系。方法 :应用CD34抗体、CD1 0 5抗体和VEGF抗体 ,采用免疫组化S -P法对我院 2 0 0 3年 3月至 6月 4 0例手术切除的胃癌患者进行血管标记和染色 ,并取 1 0例正常组织对照。结果 :应用CD34抗体、CD1 0 5抗体测得的微血管密度在癌组织内分别为 4 7.1 8± 1 7.93、2 0 .38± 8.31 ,正常组织分别为 9.30± 3.34、阴性表达 (P<0 .0 1 )。CD1 0 5标记的MVD在高中分化及低分化胃癌中比较有显著性差异 (P <0 .0 5 ) ,并且在淋巴结转移组与无淋巴结转移组胃癌中比较 ,差异显著性高于CD34标记的MVD(P <0 .0 1、P <0 .0 5 )。而两者标记的MVD与VEGF表达均呈显著正相关 (r=0 .4 70、r =0 .4 2 6)。结论 :胃癌间质内CD1 0 5标记的微血管密度特异性高于CD34标记的MVD ,两者都和VEGF表达强度与淋巴结转移密切相关 ,血管生成可能促进胃癌的发展并影响其预后 ,在胃癌进展过程中用CD1 0 5评估新血管生成可能对预测胃癌进展危险度是一个有价值的指标。可以作为一种新的肿瘤血管生成标记物用于胃癌MVD计数 ,并与VEGF均可作为预测胃癌发生转移的指标。Objective:Comparative study of the expression of CD34 and CD105 in gastric carcinoma microvessel .To investigate the correlation among microvessel density (MVD) marked with CD105 , expression of vascular endothelial cell growth factor(VEGF) and the invasion and metastasis in gastric carcinoma. Method:VEGF expression and MVD in samples of 40 gastric carcinoma and 10 normal gastric tissue collected in our hospital from 2003.3 to 2003.6 were examined by using immunohistochemical methods of S-P with anti-cd34 monoclonal antibody,anti-CD105 monoclonal antibody and anti-VEGF antibody respectively.Result:The mean value of MVD marked with CD34 and CD105 in gastric carcinoma were 47.18±17.93 and 20.38±8.31 respectively .In normal gastric tissue there were 9.30±3.34 and negtive expressing respectively(P<0.01).MVD marked with CD105 had obvious difference with histopathologic grade(P<0.05).And that was more obvious different than that marked with CD34 between in carcinoma with lymphnode involvement and carcinoma without lymphnode involvement(P<0.01、P<0.05).There was a positive correlation(r=0.470、r=0.426) between MVD (marked with CD105 and CD34) and VEGF.Conclusion:CD105 is more specificity than CD34.MVD counted with CD34 or CD105 is strongly related to the expression of VEGF and lymphnode metastasis in gastriccarcinoma .Angiogenesis might promote the progression of gastric carcinoma and affect the prognosis of the patients.Assessing neovascularization with CD105 in the process of gastriccarcinoma development may be a valueble marker for predicting the risk of gastriccarcinoma development.CD105 could be used in counting MVD as a new marker in angiogenesis.Both of the MVD and the VEGF could be used as the useful indexes reflecting the metastasis in gastric carcinoma.
