左房黏液瘤手术切除21例

摘　　要：目的　评价体外循环下手术切除左房黏液瘤的方法和临床疗效。方法　回顾性分析 2 1例体外循环下经右房 -房间隔切口行左房黏液瘤切除术患者 ,其中 4例在不阻断升主动脉、心脏跳动下完成手术 ,随访 :1月～ 9年。结果　体外循环转机 2 4～ 6 2min ,无围术期死亡及肿瘤栓塞 ,远期复发 1例 ,复发率为 4 .76 %。术后 3月心功能评价 (NYHA分级 ) :Ⅰ级 18例 ,Ⅱ级 3例。结论　体外循环下经右房 -房间隔切口切除左房粘液瘤方法简单、安全、疗效好。可以考虑不阻断升主动脉在心脏跳动下切除左房粘液瘤。Objective: To evaluate the method and clinical effect of surgical operationon left atrial myxomas under the cardiopulmonary bypass (CPB).Methods: Retrospective study was carried out oil 21 patients who received surgical operation on lefr atrial myxomas through the cut of the right atrial-al rial septum.Of the 21 cases, 4 were completed with the beating heart and the aorta wasn't cross-clamped. The follow-up ranged from 3 months to 9 years.Results:The duration of CPB ranged from 24 to 62 minutes. There was no operative death or tumor embolism during the perioperative management. The postoperative complicatiou occurred in one patient with the rate of 4.76%.And NYHA heart function class Ⅰ was in 18 patients and class Ⅱ in 3.Conclusions: The results of this study suggest that the surgical operation is a sample,safe and effective method for treating left atrial myxomas under CPB through the cut of the right atrial-atrial septum. With the consideration of that the operation can be completed on the beativg heart.
