外阴乳腺型肌纤维母细胞瘤伴小汗腺癌1例报道及文献复习

摘　　要：目的　探讨乳腺型肌纤维母细胞瘤的临床病理特点。方法　对 1例外阴乳腺型肌纤维母细胞瘤合并小汗腺癌进行常规病理、免疫组化分析。结果　外阴无痛性缓慢生长的肿块 ,镜下表面可见小汗腺癌图像 ;其下有一边界清楚的肿块 ,细胞梭形 ,束状排列 ,胶原化明显 ,肿瘤间肥大细胞丰富 ,局部见少量脂肪组织 ,梭形细胞desmin和CD34(+)。结论　发生于外阴的乳腺型肌纤维母细胞瘤是十分罕见的良性肿瘤 ,它与梭形细胞脂肪瘤形态学相似 。Objective To investigate the clinicopathologic features of the vulval soft tissue mammary type myofibroblastoma. Methods A case of vulva soft tissue myofibroblastoma with eccrine carcinoma was analyzed by light microscopy and immunohistochemistry. Results The patient was an 80 year old woman with a slowly growing painless mass measuring 5 cm in size in the vulva.In the overlying superficial dermis,there was a malignant eccrine neoplasm.Beneath this,in the submucosa,was a circumscribed mass composed of uniform spindle cells associated with dense hyaline collagen bundles and prominent stromal mast cells.It composed of a component of adipose tissue.The spindle cells characteristically expressed both desmin and CD34. Conclusion Both mammary and extramammary lesions show morphologic overlap with spindle cell lipoma and probably arise from uncommitted stem cells capable of differentiating along several mesenchymal lines.
