卵巢网状型Sertoli-Leydig细胞瘤伴异源成分1例报道并文献复习

摘　　要：目的　报道 1例卵巢网状型Sertoli Leydig细胞瘤伴异源成分并复习文献。方法　研究此瘤的病理形态学特征、免疫组化特点、临床表现及预后。结果　网状型Sertoli Leydig细胞瘤是该瘤 4种亚型中的一种 ,可伴有异源成分。网状型常发生在年轻女性 ,并且少有男性化表现。肉眼特征性改变是囊壁有息肉状突起 ,镜下特征性改变是出现网状结构。inhibin和CD99均 (+) ,性索区inhibin中度 (+)、网状区弱 (+)。结论　卵巢网状型SertoliObjective To report 1 case of retiform Sertoli Leydig cell tumor of the ovary with heterologous elements and review of the lierature. Methods The patholmorphologic features,immunohistochemical findings,clinical behaviour and prognosis were studied. Results Retiform Sertoli Leydig cell tumor was one of the four subtypes of Sertoli Leydig cell tumors.The tumor contatined the additional heterologous elements.The clinical profile presented a younger age and a lower incidence of virilization with other Sertoli Leydig cell tumors.On gross evaluation,these tumors tend to be larger and more often cystic than Sertoli Leydig cell tumors in general.A striking gross feature is the presence of large polypoid projections into cysts.Microscopically,retiform Sertoli Leydig cell tumor had a retiform pattern in addition to varying quantites of sex cord like,and gonadal stromal.Immunohistochemically CD99 were positive and inhibin was weak intensity in retiform area and moderate intensity in sex cord area.The prognosis of retiform Sertoli Leydig cell tumors has a close correlation with differentiation of tumor cells and clinical staging. Conclusions Retiform Sertoli Leydig cell tumors exhibit a wider range of morphologic appearance than any other ovarian tumor except teratomas.The diagnosis mainly relies on histopathological and immunohistochemical findings.
