高频彩超对甲状旁腺肿瘤的诊断价值

摘　　要：目的 :探讨运用高频彩超对甲状旁腺肿瘤的定位及定性诊断价值。方法 :对 1999～ 2 0 0 2年间疑有原发性甲状旁腺功能亢进的 2 1例高频彩超结果进行回顾性分析 ,全部病例经手术病理证实。结果 :1.高频彩超扫查无1例假阳性及假阴性 ,敏感性及特异性均为 10 0 % ,定位诊断精确率 10 0 % ,定性诊断正确率 95 % ;2 .甲状旁腺肿瘤均有以下声像特征 :(1)肿瘤均呈实性低回声 ,边界清楚 ;(2 )肿瘤周边及内部均有较丰富的动静脉血流信号 ,呈“花环”样改变 ;(3)肿瘤位于正常甲状腺深面 ,随吞咽动作与甲状腺同步运动。结论 :高频彩超对甲状旁腺肿瘤的诊断具有重要价值。Objective:To explore the diagnostic value of high-resolution color ultrasound (HRCU) for detecting the characteristics and location of parathyroid tumors.Methods:Twenty-one cases with suspected primary hyperparathyroidism were scanned by HRCU and confirmed by surgery and pathology during 1999-2002.Results:1.There was no false positive and false negative.The localization and characteristic diagnosing accuracies were 100% and 95%,respectively.2.The sonographic traits of parathyroid tumors were:(1)hypoechoic solid as com-pared with thyroid having clear nodular margins;(2)Rich blood flow forming a flower-like cycle features was in the peripheral nodules;(3)The tumors located in the deep portion of thyroid and moved with swallo-wing.Conclusions:It is highly valuable using HRCU to diagnose parathyroid tumors.
