进展期胃癌的X线表现与血管内皮生长因子及增殖细胞核抗原表达的关系

摘　　要：目的　探讨进展期胃癌的X线表现与血管内皮生长因子 (VEGF)及增殖细胞核抗原 (PCNA)表达间的关系。方法　分析 5 5例手术病理证实进展期胃癌的气钡双重造影和其中 3 2例的CT表现 ;应用免疫组织化学染色的方法测定切除标本中肿瘤组织VEGF及PCNA的表达结果。结果　 5 5例进展期胃癌组织VEGF及PCNA的阳性表达率分别为 65 .5 %和81.8%。双重造影肿瘤范围≥ 4区或CT示纵向直径≥ 5cm、胃窦癌伴有幽门梗阻、肿瘤与周围结构分界不清组VEGF的阳性表达率显著增高 (P <0 .0 5 )。PCNA的阳性表达率与X线表现之间无统计学显著意义。胃壁增厚的程度与淋巴结转移有相关性 (P <0 .0 5 )。结论　从细胞分子水平证明 ,气钡双重造影肿瘤范围≥ 4区、胃窦癌伴有幽门梗阻以及CT示肿瘤纵向直径≥ 5cm、与周围结构分界不清的进展期胃癌其恶性程度更大 ,提示预后较差。Objective To investigate the relationship between the radiologic appearance,expression of vascular endothelium growth factor (VEGF) and proliferating cell nuclear antigen (PCNA) in advanced gastric carcinoma. Methods Fify-five cases of double contrast radiologic appearance (32 cases with CT features) of resectable advanced gastric carcinoma proved by pathology were analyzed,using immunohistochemical staining to detect the expression of VEGF and PCNA. Results Of 55 cases advanced gastric carcinoma,the positive expression rate of VEGF and PCNA were 65.5% and 81.8% respectively. Those groups showing tumor size over four areas on double contrast imaging or over 5 cm on the longitudinal axis on CT,pylorus obstruction of gastric antrum carcinoma and unclear margins abutting upon peripheral structure had a higher positive expression rate of VEGF (P<0.05). There was no relationship between the positive expression rate of PCNA and radiologic appearance. The degree of thickness of gastric wall was closely related to lymphatic metastasis (P<0.05). Conclusion The results indicates those advanced gastric carcinoma with double contrast showing tumor size over four areas,pylorus obstruction of gastric antrum carcinoma and CT signs of tumor size over 5 cm on the longitudinal axis,unclear margins abutting upon peripheral structure had a higher degree of malignancy and a poor prognosis.
