163例颌面颈部恶性淋巴瘤临床分析

摘　　要：目的:回顾分析颌面颈部恶性淋巴瘤的临床特点,治疗和预后。方法:对163例颌面颈部恶性淋巴瘤患者的临床资料及随访情况进行回顾性研究。结果:恶性淋巴瘤163例中,男:女=1.86:1,中位年龄43.2岁。发病部位依次为颈部,腭部,腮腺及牙龈,首次就诊误诊率47.9％。主要采用单纯化疗及综合治疗。随访病人存活时间超过10年8例,超过5年31例,死亡病例平均存活20.6月,中位生存期为12个月。结论:颈部恶性淋巴瘤首诊中易被误诊;组织活检以明确诊断并制定相应的治疗计划至关重要;化疗及综合治疗是本病的主要治疗手段。Objective:To study retrospectively the clinical characteristics, treatment and prognosis of malignant lym-phoma in maxillofacial and neck region. Method: Clinical data of 163 cases were obtained from patient files. Each patient' s complete record was reviewed. Result: Of a total of 163 malignant lymphomas (10 Hodgkin' s disease, 153 non -Hodgkin' s lymphoma) recorded in a 20 - year period, the ratio of male/female was 1.86:1 (106/57) and median age of the patients was 43. 2(range, 2-81 years old). Affected anatomic sites included: neck (57), palate (35), salivary glands (21), gingival (20), mandible (8), maxilla(6), and other (16). The most common presenting symptom was a non - painful mass. 68 patients had stage Ⅰ, 56 patients stage Ⅱ, 31 patients stage Ⅲ, and 8 patients stage Ⅳ. Treatment included chemotherapy , radiation and so on. Treatment trends indicate a shift towards multimodal therapy. Follow - up ranged from 6 to 168 months. The total survival rate over 10 years was 4.9%, over 5 years was 19.0%. The survival time was 20. 6 months in average. The median survival time is 12 months. Conclusion:(1)Malignant lymphoma was often misdiagnosed for other common oral diseases. (2)Early diagnosis depended on a thorough examination and biopsies from suspected lesions. (3)Chemotherapy and multimodal therapy were chief treatment methods for malignant lymphoma in maxillofacial and neck region, (4)If discovered and treated early, malignant lymphoma in maxillofacial and neck regions has an better prognosis.
