儿童后发性白内障Nd:YAG激光治疗的并发症及其对策分析

摘　　要：目的　探讨 Nd:YAG激光治疗儿童后发性白内障的临床效果 ,重点分析激光治疗中出现的并发症及其处理对策。方法　对儿童后发性白内障 4 0例 (5 2只眼 )行 Nd:YAG激光后囊切开术 ,术后随访 3～ 12月 ,观察并分析术后视力变化和并发症情况。结果　激光截囊成功率为 10 0 % ,术后视力有不同程度提高。并发症包括人工晶状体损伤 (3.8% )、玻璃体前界膜破裂 (15 .4 % )、葡萄膜炎反应 (11.5 % )、高眼压 (9.6 % )等。结论　 Nd:YAG激光治疗儿童后发性白内障简便有效 ,能明显提高视力 ,只要操作得当 ,聚焦准确 ,掌握好击射能量等 。Objective To discuss the clinical effect of capsulotomy with Nd:YAG laser in children's after cataract.To analyze the complication and it's countermeasure.Methods Capsulotomy with Nd:YAG laser were done in 40 cases (52 eyes) of children's after cataract.All cases were followed up for 3～12 months.The postoperative visual acuity and complication were observed and analyzed.Results Successful casulotomy rate was 100%.The visual acuity was improved to varying degrees in all cases.Postoperative complication included damnification of IOL(3.8%),disruption of anterior limiting lamina in vitreous (15.4%),uveitis (11.5$),postoperative ocular hypertension (9.6%) and so on.Conclusion It is a very simple and effective method to make capsulotomy with Nd:YAG laser in children's after cataract.It can improve visual acuity remarkably.The postoperative complication could be avoided effectively if the operation would be done accuraterly.
