中、老年高度近视眼透明晶状体超声乳化吸出联合人工晶体植入的临床观察

摘　　要：目的　评价中、老年高度近视眼经超声乳化吸出透明晶状体联合植入折叠式人工晶体术后屈光矫正治疗的有效性和安全性。方法　 3 4例 (5 5眼 )高度近视眼行表面麻醉下经颞侧透明角膜切口 ,透明晶状体超声乳化吸出联合折叠式人工晶体植入 ;平均年龄 5 4 7岁 ,术前矫正视力 0 1～0 6,平均近视度数 -14 3 1± 4 73D ,随访期 8～ 3 7月 ,平均 2 8 5月。结果　术后所有患者裸眼及矫正视力都有不同程度的提高 ,术后三月 ,最佳矫正视力 0 2～ 1 0 ;49眼≥ 0 5 (89 1% ) ;术后平均近视度数 -1 82± 0 5 2D ;术前与术后三月相比较 ,角膜散光度数差异无显著性 (t =1 813 ,P >0 0 5 )术后 4眼 (7 3 % )发生晶状体后囊膜混浊 ,无其他手术并发症的发生。结论　对中、老年高度近视眼进行超声乳化吸出透明晶状体联合植入折叠式人工晶体屈光矫正治疗是安全、有效的。但术前应进行细致的眼底视网膜检查。该屈光性手术术中、术后并发症低 ,手术疗效满意。Objective To assess phacoemulsification with foldable intraocular lens implantation as an effecgtive,safe technique for the correction of high myopia in middle and old people.Methods A series of 55 eyes of 34 high myopia patients in middle and old people had clear lens extraction by phacoemulsification and foldable intraocular lens implantation through clear cornea incision.The average age of patients was 54.7 years(42～68 years).The preoperative corrected visual acuity was 0.1～0.6.The mean preoperative spherical equivalent was -14.31±4.73D.The mean postoperative follow-up was 28.5 months ±3.81(8～37 months).Results The uncorrected visual acuity and the corrected visual acuity was improved in all cases postoperatively.The best corrected visual acuity was from 0.2 to 1.0 3m postoperatively.49 eyes having achieved the best corrected visual acuity of 0.5 or better were accounted for 89.1%.The mean postoperative spherical equivalent was -1.82±0.52D.There were no significant difference between preoperative and 3m postoperative mean corneal astigmatism(t=1.813,P>0.05).The 4 eyes of all case occurred posterior capsular opacification(7.3%).No other complications occurred.Conclusions phacoemulsification with foldable intraocular lens implantation for the correction of high myopia in middle and old people is a safe,effective procedure as a refractive surgery.It is necessary for careful examination of retinal periphery.The technique is associated with low incidence of intraoperative and postoperative complication and good outcome of visual acuity.
