向透明角膜切口超声乳化手术的过渡

摘　　要：目的　探讨向透明角膜切口超声乳化手术过渡过程中可能遇到的问题和相应的手术技巧。方法　对最初实施的3 0例透明角膜切口白内障超声乳化手术进行回顾性分析 ,以同期完成的 3 0例巩膜隧道切口超声乳化手术作为对照。结果　透明角膜切口组的总手术时间明显短于巩膜隧道切口组 (P <0 0 1) ,并有较好的美容效果和较轻的术后炎症反应。透明角膜切口组有 2眼术中出现了球结膜“气球样”水肿 ,另有 2眼需间断缝合以保持切口水密状态。结论　在具有较丰富的巩膜隧道切口超声乳化手术经验的基础上 ,正确掌握透明角膜切口的制作方法是实现安全过渡的关键。Objective To discuss the strategy and surgical skills of transition from scleral tunnel incision to clear corneal incision.Methods The initial 30 cases of clear corneal phacoemulsification were reviewed,while another 30 cases who had underwent scleral tunnel phacoemulsifcation during the same period were selected as the control group.Results The total surgical time was significantly shorter in the clear corneal incision group than that in the scleral tunnel incision group ( P <0.01).The clear corneal incision group had a better aesthetic effect and milder inflammatory response than the control group.In the clear corneal incision group,two eyes developed ballooning of the conjunctiva during phacoemulsification,and single interrupted sutures were engaged to keep the incisions watertight in another two eyes.Conclusion The adequate experience in scleral tunnel incision phacoemulsification and proper construction of the clear corneal incision are the key to the safe technical transition.
