D-二聚体在口腔颌面部恶性肿瘤诊治中的意义

摘　　要：目的　了解口腔颌面部恶性肿瘤患者血浆D -二聚体在术前诊断、手术治疗效果及预后判断中的意义。方法　追踪本科收治的口腔颌面部恶性肿瘤患者 31例 ,用ELISA法检测患者术前、术后、以及定期随访时的血浆D -二聚体值 ,并且与健康成年人相比较。结果　肿瘤组术前测定血浆D -二聚体值阳性率为 96 .8% ,平均为 1.3mg/L ,术后检测血浆D -二聚体值阳性率为 9.6 8% ,平均为 0 .34mg/L。术前组明显高于对照组 (P <0 .0 0 1) ,也明显高于术后组 (P <0 .0 5 )。结论　血浆D -二聚体浓度的异常升高 ,与口腔颌面部恶性肿瘤的发生、发展 ,以及肿瘤的大小、分期有相关性。可以通过对血浆D -二聚体值的监测来辅助诊断口腔颌面部恶性肿瘤、评估手术治疗效果、判断预后。Objective To investigate the significance of plasma D-dimer of the patients with oral and maxillofacial malignant tumor at diagnosis before surgery, surgical treatment result evaluation and the judgment prognosis. Methods We tracked with 31 cases with oral and maxillofacial malignant tumor in our department. The value of plasma D-dimer with ELISA examination before and after surgery, and further consultation on schedule, and compare with health adults were perfored Results The positive rates of the value of plasma D-dimer of before surgery group was 96.8%, average was 1.3mg/L, and after surgery group was 9.68%, average was 0.34mg/L. Before surgery group was obvious higher than normal control group (P<0.001), and also obvious higher than after surgery group (P<0.05). Conclusion The result indicated that the unusual raise of the consistency of plasma D-dimer correlated with the oral and maxillofacial malignant tumor's occurrence, development, the size of the tumor and the tumor's stage. So we can track the plasma D-dimer as a complement to diagnosis oral and maxillofacial malignant tumor, to evaluate surgical curative effects and to estimate prognosis.
