摘  要:目的 探讨p2 1WAF1基因多态性及其蛋白表达与乳腺癌组织学分级的关系。方法 分别采用聚合酶链反应单链构象多态性技术 (PCR -SSCP)和免疫组织化学LSAB法检测 10 0例乳腺癌组织p2 1WAF1基因多态性和蛋白表达。结果 具有p2 1WAF1DNA多态性和无多态性的乳腺癌组织p2 1WAF1蛋白表达阳性分别为 18例 (10 0 % )和 32例 (39 0 % ) ,前者明显高于后者 (P <0 0 1) ;在乳腺癌组织腺管形成 1,2 ,3组中具有p2 1WAF1DNA多态性改变和蛋白阳性的病例数分别为 1(5 0 % ) ,6 (14 0 % ) ,11(2 9 7% )和 7(35 0 % ) ,2 0 (4 6 5 % ) ,2 3(6 2 2 % ) ;在细胞核多形性 1,2 ,3组中具有p2 1WAF1DNA多态性改变和蛋白阳性的病例数分别为 1(5 9% ) ,2 (4 8% ) ,15 (36 6 % )和 5 (2 9 4 % ) ,15(35 7% ) ,30 (73 2 % ) ;在组织学分级 1,2 ,3组中具有p2 1WAF1DNA多态性改变和蛋白阳性的病例数分别为 3(8 8% ) ,4 (8 7% ) ,11(5 5 0 % )和 14 (4 1 2 % ) ,2 1(4 5 7% ) ,15 (75 0 % ) ,在核分裂象计数 1(<5个 ) ,2 (5~ 10个 ) ,3(>11个 )组中p2 1WAF1蛋白阳性例数分别为 7(36 8% ) ,19(4 3 2 % )和 2 4 (6 4 8% )。以上各指标组间比较差异有显著性 ,p2 1WAF1DNA多态性与其蛋白表达呈正相关关系 (P均 <0 0 1)。Objective To explore the association of p21 WAF1 gene polymorphism and its protein expression with the histological gradings of breast carcinomas. Methods Polymerase chain reaction single-strand conformation polymorphism technique (PCR-SSCP) and LSAB immunohistochemistry were used to identify p21 WAF1 gene polymorphisms and gene protein expessions respectively on paraffinembedded tissues of 100 cases of breast carcinomas and 40 benign breast diseases as control. Results The increased protein expression of p21 WAF1 gene and DNA polymorphisms were detected in 18 cases (100%), compared with 32 cases (39.0%) with incereased p21 WAF1 gene protein expression only but without DNA polymorphisms in breast carcinomas. The difference between groups was statistically significant(P<0.01). In groups of tubule formation grade Ⅰ,Ⅱ,Ⅲ of breast carcinomas, tissues with p21 WAF1 DNA polymorphisms were found in 1(5.0%),6(14.0%) and 11(29.7%), while tissues with positive p21 WAF1 protein expression only were found in 7(35.0%), 20(46.5%) and 23(62.2%) respectively. The positive p21 WAF1 DNA polymorphisms in grade Ⅰ,Ⅱ,Ⅲ of nuclear polymorphism grading were present in 1(5.9%),2(4.8%) and 15(36.6%) cases, while 5(29.4%),15(35.7%), and 30(73.2%) cases had positive p21 WAF1 gene protein expression. In groups of histological grade Ⅰ,Ⅱ,Ⅲ of breast carcinomas, tissues with p21 WAF1 DNA polymorphisms were found in 3(8.8%),4(8.7%) and 11(55.0%) cases, while tissues with positive p21 WAF1 protein expression only were found in 14(41.2%),21(45.7%) and 15(75.0%) cases respectively. In grade Ⅰ(<5 mitosis), Ⅱ(5~10 mitosis) and Ⅲ(> mitosis) of mitosis of breast carcinomas, the prsitive protein expressions were found in 7(36.8%), 19(43.2%) and 24(64.8%) cases. The above parameters had significant differences among different groups respectively, p21 WAF1 DNA polymorphism were positively correlaled with p21 WAF1 protein expression(P<0.01).
