胃癌术后联合腹腔灌注化疗的临床观察

摘　　要：目的:探讨胃癌术后联合腹腔灌注化疗对改善预后的作用。方法:胃癌术后经病理证实病变浸润至浆膜层以外和淋巴结转移的患者79例,随机分为A、B、C、D组,A组接受静脉注射亚叶酸钙和5-FU、羟基喜树碱、顺铂;余各组用药与A组相同,同时分别用亚叶酸钙和5-FU、羟基喜树碱、顺铂腹腔灌注,每周期2次。各组均治疗6个周期。结果:A组的3年无瘤生存率为45.00%(9/20),低于术后联合腹腔灌注化疗B组(83.33%,15/18)、C组(86.36%,19/22)、D组(73.68%,14/19)(P<0.05);3年生存率A组(60.00%,12/20)也低于B组(94.44%,17/18)、C组(95.45%,21/22)、D组(84.21%,16/19)(P<0.05)。腹泻、腹痛、不全性肠梗阻等并发症各组无明显差异。结论:胃癌术后联合腹腔灌注巩固化疗有利于改善预后。Objective:To explore the effect of united abdominal infusion in assistant chemotherapy for postoperative patients with gastric carcinoma.Method:79cases of patients re-ceived radical resection of gastric cancer with confirmed pathologically infiltration out of placenta percreta and/or metastasis of lymph node were divided randomly to A,B,C,D groups.In group A,calcium leueovorin and5-Fu,hydroxycamptothecin(HCPT),cisplatin administered through injec-tion of vein;Other groups were treated by same drugs and the way with one of those agents re-spectively in abdominal infusion.Every group received treatment for6cycles.Results:Survival rates without neoplasm of3rd year in group A (45%,9/20)was lower than that in group B(83.33%,15/18),group C(86.36%,19/22),in group D(73.68%14/19)(P<0.05);survive rates of3rd in group A(60%,12/20)was inferior respectively group B(94.44%,17/18),group C(95.45%,21/22)or group D(84.21%,16/19)(P<0.05).There were not markedly difference among the groups in the complication of diarrhea,abdominalgia and incomplete intestinal obstruction.Con-clusion:It was significant effect for improving prognosis with united abdominal infusion in assis-tant chemotherapyfor postoperative patients with gastric cancer.
