重组人肿瘤坏死因子治疗87例中晚期恶性肿瘤

摘　　要：目的:评价重组人肿瘤坏死因子(rnhTNF)对中晚期恶性肿瘤的临床疗效,观察rnhTNF的不良反应。方法:随机分为联合组(A组)48例和单纯化疗组(B组)22例;另设单纯TNF组(C组)17例。A组:化疗同时肌肉注射rnhTNF400万U/m2,第l～7天和第11～17天。21天为一周期,连用两个周期。B组:仅给予化疗,剂量和化疗周期同A组。C组:仅肌肉注射rnhTNF治疗,剂量和给药方法同A组。结果:A组有效率(CR+PR)为18.75%(9/48),B组为0(0/22),C组为0(0/17);A组与B组治疗前、后KPS评分和治疗前、后KPS评分差值,均无明显变化。A组第1、2周期与TNF有关的不良反应发生率为87.50%(42/48)和91.70%(44/48),C组与TNF有关的不良反应发生率为100%(17/17),但两组差异无显著性。结论:rnhTNF联合化疗治疗肺癌、头颈部癌、消化道癌、泌尿系恶性肿瘤等有一定的疗效,无严重的不良反应。Objective:To valuate the effect of recombinant new human tumor necrosis factor(rnhTNF)on advanced cancers,and observe the reverse response of rnhTNF.Methods:Eighty-sev-en cases with advanced cancer were divided into3groups:those in group A were giv en4MU/m 2 rnhTNF im once a day in day1～7and day11～17,meanwhile chemotherapy were medi cated.In group B only chemotherapy were given at the same dosage as group A.In group C were merely given rnhTNF at the same dosage as group A.Rusults:the response rate(CR+PR)in group A,B and C are18.75%(9/48),0%(0/22)and0%(0/17),respectively.Karnofsky sta tus is insignificantly different between group A and B either before or after the treatment.Fever and pain in injected site,caused by rnhTNF,occurred in group A at the percentage of91.7and,100%in group C.Conclusion:rnhTNF combined with chemotherapy is effective for advanced can cers.
