特制人工假体在四肢骨肿瘤保肢治疗中的临床应用

摘　　要：目的 :探讨特制人工关节在骨肿瘤保肢术中的应用价值。方法 :对 5 3例骨肿瘤切除、特制人工假体置换重建骨缺损的随访结果进行分析 ,包括术后疗效、生存情况、功能状况、并发症及处理。肿瘤包括恶性骨肿瘤 2 3例 ,良性骨肿瘤 30例。结果 :2 3例恶性骨肿瘤 :5年无瘤生存率 34 8% (8/ 2 3) ,5年生存率 5 2 2 % (12 / 2 3) ,局部复发率 17 4 % (4/ 2 3) ,最终保肢率 82 6 %。 30例良性骨肿瘤并发症发生率为 4 0 % (8/ 2 0 ) ,总的保肢率为 85 % (17/ 2 3)。结论 :特制人工假体置换可用于骨肿瘤的保肢治疗 ,但应进一步改善假体设计、固定、软组织重建 ,降低并发症。Objective:To evaluate the application of special made prosthetic replacement in extremity salvage treatment of bone tumors.Methods:53 patients with bone tumor treated with neoplasms resection and special made prosthetic replacement reconstruct bone losses were reviewed.Their postoperative effects,survivorship,extremity function,complication and its management were analyzed.There were 23 malignant tumors and 30 benign tumors.Results:In patients with malignant bone tumors in 23 cases,8(34 8%) belonged to disease free survival (DFS) at 5 years after surgery.DFS probability at 5 years was 52 5% (12/23).Local recurrence rate was 17 4% (4/23) and extremity salvage rate was 82 6%.In 30 benign bone tumors,complication occurred in 40% of the patients,and extremity salvage rate was 85%.Conclusion:Special made prosthetic replacement can be used in extremity salvage surgery of patients with bone tumors.Further improvement in design,fixation and soft tissue reconstruction technique of the prosthetic is needed for reducing the complication.
