摘 要:目的通过对儿童基础代谢率与人体组成关系的研究,为临床基础代谢率的应用提供资料。方法对114名儿童进行基础代谢能量消耗和人体组成(脂肪组织和去脂组织)测定,分析其相关关系。结果基础代谢的能量消耗肥胖组儿童高于非肥胖组儿童,两者比较差异有显著意义(P<0.05)。用去脂组织(FFM)和脂肪组织(FM)校正后,肥胖组与非肥胖组无显著性差异(P>0.05),但男、女儿童间仍有显著性差异(P<0.05)。基础代谢率与FFM呈显著正相关(r=0.895,P<0.001),FFM可解释基础代谢率变化的80%,是影响基础代谢率的主要因素,性别和FM分别可解释其2.4%和2.7%。用去脂组织、脂肪组织和性别预测基础代谢能量消耗的回归方程:基础代谢率kJ/h=100.984+3.243×去脂组织+15.333×性别+1.533×脂肪组织(性别:男童为1,女童为0)。结论用FFM和FM对基础代谢率校正以后,基础代谢的能量消耗量肥胖儿童与非肥胖儿童无显著性差异。FFM是影响儿童基础代谢率的主要因素,FM对基础代谢率也有一定影响。Objective To study the correlation between children′s basal metabolic rate(BMR)and body composition and provide basal data for the clinic application of BMR. Methods BMR of 114 children was measured by using open-circuit indirect calorimetry in a full-time boarding school in Guangzhou suburb. At the same time, skin-fold thickness was measured by using standard methods. According to formulas,the percentage of body fat, fat mass(FM)and fat-free mass(FFM)were calculated. Results The BMR was significantly(P < 0.05)higher in the obese(males: 266.02 kJ/h ± 35.44 kJ/h;females:237.35 kJ/h ± 23.08 kJ/h)than in the nonobese(males: 219.99 kJ/h ± 20.75 kJ/h;females:205.27 kJ/h ± 17.77 kJ/h)children. After BMR was adjusted for FFM+FM, the difference in BMR between the obese and the nonobese children disappeared(P > 0.05), but the difference between the genders still remained significant(P < 0.05). BMR was positively correlated with FFM(r = 0.895,P < 0.001=. The main determinant of BMR was FFM. In the whole cohort, FFM explained 80% of the variation in BMR, followed by gender and FM adding further 2.4%, 2.7% to the predictability of BMR, respectively. The equation of estimating BMR from FFM, FM and gender was as follows: BMR(kJ/h)=100.984+3.243×FFM+15.333×gender+1.533×FM(Male=1, Female=0). Conclusions BMR of obese and nonobese children was not different when adjusted for body composition(FFM+FM). FFM was a main factor affecting BMR, however, FM was also a significant factor.
