摘  要:目的 探讨慢性HBV感染相关的肝癌病人外周血树突状细胞 (DC)亚群的特点和临床意义。方法 选择 2 6例慢性HBV感染相关的肝癌病人进行研究 ,2 3例慢性乙型肝炎病人和 2 0例健康人作为对照。流式细胞仪检测外周血DC亚群和淋巴细胞亚群。ELISA法检测外周血单个核细胞 (PBMC)经灭活 1型单纯疱疹病毒 (HSV 1)刺激后产生IFN α的能力。结果 与健康对照相比 ,慢性HBV感染相关的肝癌病人外周血DC1和DC2百分比和绝对细胞数降低 (P <0 0 5 ) ;DC1/DC2比率无显著差别 ;PBMC经灭活HSV 1刺激后产生IFN α的能力降低 (P <0 0 5 ) ;NK细胞绝对细胞数降低(P <0 0 5 )。结论 慢性HBV感染相关的肝癌病人外周血DC1和DC2数量减少、功能减低 ,且伴有NK细胞绝对数目的减少 ,这些可能是导致肝癌病人机体抗肿瘤免疫反应下降的重要原因。Objective To investigate the characteristics and clinical significance of peripheral blood dendritic cell (DC) subsets in patients with primary hepatocellular carcinoma (HCC). Methods Twenty-six HCC patients were enrolled for study. Twenty three patients with chronic HBV infection and twenty healthy individuals were entered as controls. Peripheral blood DC subsets and lymphocyte subsets of patients and healthy subjects were analyzed by flow cytometry. Interferon-α(IFN-α) production of peripheral blood mononuclear cells (PBMC) induced by inactivated herpes simplex virus type 1 (HSV-1) was measured by a human IFN-α ELISA kit. Results Compared with healthy subjects, patients with HCC had a significantly decreased proportion and absolute number of DC1 and DC2 (P<0.05). The ratio of DC1/DC2 was not statistically different between patients with HCC and healthy subjects. The ability of PBMCs to produce IFN-α after inactivated-HSV-1 stimulation was significantly decreased in patients with HCC (P<0.05). The absolute number of peripheral blood NK cells in patients with HCC was dramatically decreased (P<0.05). Conclusion Patients with HBV-associated hepatocellular carcinoma may have a significantly decreased number and impaired function of peripheral blood DC1, DC2 and NK cells, contributing to functional deficiency of host innate and adaptive immune responses in patients with HCC.
